Abstract:Background: The purpose of this study was to investigate the diagnosis and treatment experience of traumatic duodenal ruptures in children. Methods: Clinical data were collected from four children suffering from a traumatic duodenal rupture who were admitted to and treated by our hospital from January 2012 to December 2020. The early diagnosis and treatment, surgical plan, postoperative management, complications, and prognosis of each child were analyzed. The key points and difficulties of the diagnosis and treatment for this type of injury are summarized. Results: One child had an extreme infection caused by drug-resistant bacteria, which resulted in severe complications, including wound infection, dehiscence, and an intestinal fistula. One child developed an anastomotic stenosis after the duodenostomy, which improved following an endoscopic balloon dilatation. The other two children had no relevant complications after their operations. All four patients were cured and discharged from hospital. The average hospital stay was 48.25 ± 26.89 days. The follow-up period was 0.5 to 1 year. No other complications occurred, and all children had a positive prognosis. Conclusions: The early identification of a duodenal rupture is essential, and surgical exploration should be carried out proactively. The principles of damage-control surgery should be followed as much as possible during the operation. Multidisciplinary cooperation and management are both important to reduce the occurrence of postoperative complications and improve cure rates.

Abstract:Duodenal trauma is uncommon but nowadays seen more and more frequently due to the increased automobile accidents and violent events. The management of duodenal trauma can be complicated, especially when massive injury to the pancreatic-duodenal-biliary complex occurs simultaneously. Even the patients receive surgeries in time, multiple postoperative complications and high mortality are common. To know and manage duodenal trauma better, we searched the recent related literature in PubMed by the keywords of duodenal trauma, therapy, diagnosis and abdomen. It shows that because the diagnosis and management are complicated and the mortality is high, duodenal trauma should be treated in time and tactfully. And application of new technology can help improve the management. In this review, we discussed the incidence, diagnosis, management, and complications as well as mortality of duodenal trauma.

Abstract:With the development of modern traffic,the incidence of duodenal trauma has been increased year after year.Some progress has been achieved in its diagnosis and treatment.Upper gastrointestinal radiology or CT may be the best diagnostic measurement of blunt duodenal trauma,while laparoscopy is not very valuable.Exploratory laparotomy remains the ultimate diagnostic method for a high suspicion of duodenal injury.The majority of duodenal injuries may be managed by simple repair.High-risk duodenal injuries is followed by a high incidence of suture line dehiscence,which should be treated by duodenal diversion.Pancreaticoduodenectomy should be considered only if no alternative is available.

Abstract:AIM: To retrospectively evaluate the preoperative diagnostic approaches and management of colonic injuries following blunt abdominal trauma.METHODS: A total of 82 patients with colonic injuries caused by blunt trauma between January 1992 and December 2005 were enrolled. Data were collected on clinical presentation, investigations, diagnostic methods, associated injuries, and operative management. Colonic injury-related mortality and abdominal complications were analyzed.RESULTS: Colonic injuries were caused mainly by motor vehicle accidents. Of the 82 patients, 58 (70.3%) had other associated injuries. Laparotomy was performed within 6 h after injury in 69 cases (84.1%), laparoscopy in 3 because of haemodynamic instability. The most commonly injured site was located in the transverse colon. The mean colon injury scale score was 2.8. The degree of faecal contamination was classified as mild in 18 (22.0%), moderate in 42 (51.2%), severe in 14 (17.1%), and unknown in 8 (9.8%) cases. Sixty-seven patients (81.7%) were treated with primary repair or resection and anastomosis. Faecal stream diversion was performed in 15 cases (18.3%). The overall mortality rate was 6.1%. The incidence of colonic injury-related abdominal complications was 20.7%. The only independent predictor of complications was the degree of peritoneal faecal contamination (P = 0.02).CONCLUSION: Colonic injuries following blunt trauma are especially important because of the severity and complexity of associated injuries. A thorough physical examination and a combination of tests can be used to evaluate the indications for laparotomy. One stage management at the time of initial exploration is most often used for colonic injuries. (c) 2007 The WJG Press. All rights reserved.

Abstract:This is an article about paediatric duodenal hematoma, which suggests that conservative treatment for traumatic duodenal hematoma is effective, with most cases recovering within 4 weeks after injury. Background Paediatric traumatic duodenal hematoma is a rare type of blunt abdominal injury for which treatment strategies are controversial. This study aimed to evaluate the application value of nonoperative management of paediatric duodenal hematoma caused by trauma. Methods A retrospective analysis was conducted on patients with duodenal hematoma with a confirmed history of abdominal trauma admitted to our hospital between January 2010 and December 2022. General patient information, including age, sex, cause of injury, clinical manifestations, and treatment outcomes, was recorded. Results A total of 11 cases were included in this study, with 6 males and 5 females. School‐age children (≥6 years) accounted for 72.7% (8/11) of the cases. Bicycle handlebar injuries accounted for 63.6% (7/11) of cases. Among these cases, 5 (45.5%) were classified as American Association for the Surgery of Trauma grade I, while the remaining were classified as grade II. The median history time was 1 day (range: 2 h–12 days). All patients were successfully treated using non‐operative methods. The median time to oral feeding after injury was 17 days (range: 9–32 days). Oral feeding was initiated within 2 weeks in 2 patients (18.2%), within 3 weeks in 6 patients (54.5%), and within 4 weeks in 10 patients (90.9%). Conclusion Paediatric traumatic duodenal hematoma is more common in school‐aged children, mainly due to bicycle handlebar injuries. Nonoperative treatment is proven to be safe and effective, with duodenal obstruction symptoms typically resolving within 4 weeks.

Congenital duodenal obstruction in neonates: a decade’s experience from one center

Abstract:Background Congenital duodenal obstruction (CDO) is one of the most common anomalies in newborns, and accounting for nearly half of all cases of neonatal intestinal obstruction. This study aimed to review our single-center experience in managing congenital duodenal obstruction while evaluate the outcomes. Methods We conducted a retrospective analysis of the records of all neonates dianogsed with congenital duodenal obstruction admitted to our center between January 2003 and December 2012. We analyzed demographic criteria, clinical manifestations, associated anomalies, radiologic findings, surgical methods, postoperative complications, and final outcomes. Results The study comprised 287 newborns (193 boys and 94 girls). Birth weight ranged from 950 g to 4850 g. Fifty-three patients were born prematurely between 28 and 36 weeks’ gestation. Malrotation was diagnosed in 174 patients, annular pancreas in 66, duodenal web in 55, duodenal atresia or stenosis in 9, preduodenal portal vein in 2, and congenital band compression in 1. Twenty patients had various combinations of these conditions. Presenting symptoms included bilious vomiting, dehydration, and weight loss. X-rays of the upper abdomen demonstrated the presence of a typical double-bubble sign or air-fluid levels in 68.64% of patients, and confirmatory upper and/or lower gastrointestinal contrast studies were obtained in 64.11%. Multiple associated abnormalities were observed in 50.52% of the patients. Various surgical approaches were used, including Ladd’s procedure, duodenoplasty, duodenoduodenostomy, duodenojejunostomy, or a combination of these. Seventeen patients died postoperatively and 14 required re-operation. Conclusions Congenital duodenal obstruction is a complex entity with various etiologies and often includes multiple concomitant disorders. Timely diagnosis and aggressive surgery are key to improving prognosis. Care should be taken to address all of the causes of duodenal obstruction and/or associated alimentary tract anomalies during surgery.
Failed primary repair of blunt duodenal injury managed by tube duodenostomy, gastrojejunostomy and a feeding jejunostomy: a case report

Abstract:Background: The worldwide increase in road traffic crashes and use of firearms has increased the incidence of duodenal injuries. Upper gastrointestinal radiological studies and computed tomography (CT) in resource settings may lead to the diagnosis of blunt duodenal injury. Exploratory laparotomy remains the ultimate diagnostic test if a high suspicion of duodenal injury continues in the face of absent or equivocal radiographic signs. Although the majority of duodenal injuries may be managed by simple repair, high-risk duodenal injuries are followed by a high incidence of suture line dehiscence and should be treated by duodenal diversion. Case presentation: We report a case of a failed primary repair of a blunt injury to the second part of the duodenum (D2) in a 24-year-old African man. This was successfully managed by a tube duodenostomy, a bypass gastrojejunostomy and a feeding jejunostomy in a low resource setting. Conclusions: Detailed knowledge of the available operative choices in duodenal injury and their correct application is important. When duodenal repair is needed, conservative repair techniques over complex reconstructions should be utilised. The technique of tube duodenostomy can be successfully applied to cases of large defects in the second part of the duodenum (D2), failed previous repair attempts and with defects caused by different aetiology. It may remain especially useful as a damage-control procedure in patients with multiple injuries, significant comorbidities and/or haemodynamic instability.
Diagnosis and treatment of blunt diaphragmatic rupture:an analysis of 36 cases

Abstract:Objective: To study the clinical diagnosis and surgical intervention of blunt traumatic diaphragmatic rupture.Methods: The clinical data of 36 patients with traumatic diaphragmatic rupture from January 2000 to December 2008 were retrospectively reviewed,including etiology,traumatic position,complicated injury,score of injury severity,diagnosis course and surgical treatment.In the series,there were 29 cases of left-diaphragmatic rupture,6 right diaphragmatic rupture and 1 case in bilateral.All the patients received operation.Thoracotomy was performed in 6 patients,laparotomy in 30,thoracotomy plus laparotomy in 2 and combined thoraco-laparotomy in 1 case.Results: Thirty-four cases were cured.One patient died of liver rupture,hemorrhagic shock,wound shock and DIC,and one of severe cerebral injury and MODS.Conclusion: Diaphragmatic rupture caused by blunt injury could lead to severe thoracoabdominal injury.In spite of high mortality rate,the condition is usually under diagnosed.Early diagnosis and active operation including definite treatment of complicated injures are the keys to increase survival rate and reduce mortality.
Pediatric Pyloric Transection: An Unusual Injury Following Blunt Abdominal Trauma

Abstract:BACKGROUND The pediatric population, due to its distinct anatomy and physiology, often presents with unique mechanisms of trauma, leading clinicians to encounter diverse and sometimes unexpected injuries. Whether these injuries result from blunt or penetrating trauma, they may involve intra-abdominal organs in pediatric patients. Additionally, there are occasional occurrences where injuries affect rare sites such as the pylorus in an isolated manner within this age group. Clinicians must be prepared to address a wide range of injury patterns to ensure optimal outcomes for pediatric patients experiencing trauma to intra-abdominal structures such as the pylorus. CASE REPORT We report a 19-month-old boy who presented with abdominal pain, crying, and repeated vomiting of gastric contents after a wardrobe fell on his upper torso. His vital signs were stable except that he was tachycardiac. Upon investigation, abdominal computed tomography (CT) revealed pneumoperitoneum, free peritoneal fluid, and inflammatory changes in the intestinal wall, suggesting hollow viscus injury. Exploratory laparotomy was performed, and complete transection of the pyloric area of the stomach was identified. The pancreatic and biliary ducts were intact. On postoperative day 5, an upper gastrointestinal (UGI) contrast study prior to initiating oral feeding was done and showed normal findings with no contrast leakage. His postoperative course was unremarkable. CONCLUSIONS Isolated pyloric injuries following blunt trauma are rare with no known case reports in pediatric age group. High morbidity and mortality rates can result from traumatic gastrointestinal injuries including the involvement of pylorus. Therefore, accurate diagnosis and prompt management are essential for an improved outcome.

Abstract:Objective To improve the level of diagnosis and treatment to the small intestine rupture caused by closed abdominal injury.Methods Retrospective analysis was carried in 38 patients with small intestine rupture caused by closed abdominal injury.Results This group of patients was treated with operation.35 cases were cured and 3 cases were dead.8 cases were complicated with incision infection,1 case with early adhesive intestine obstruction.Conclusion It is much key to raise the cure rate,decrease the complications and reduce the death rate by early diagnosis,prompt operation and attention to whole treatment.

Abstract:Traumatic bladder injuries are rare, comprising only 0.78% to 1.6% of blunt abdominal injuries, with 85% of them being blunt injuries. [ 1 ] The rarity of bladder injuries is primarily attributed to the protective pelvic structure, typically stemming from high-energy trauma. Clinical signs of traumatic bladder injuries are nonspecific and mainly involve gross hematuria, suprapubic pain/tenderness, and urinary difficulties. [ 2 , 3 ] Clinicians can easily diagnose and treat bladder injuries when typical clinical signs are present, but patients with atypical symptoms, especially delayed bladder rupture, may be overlooked or misdiagnosed, potentially impacting treatment efficacy. In this study, we present a rare and easily overlooked case of delayed intraperitoneal bladder rupture resulting from blunt trauma. Ethical approval for this study was obtained from the ethics committee of the Chongqing Emergency Medical Center. The patient and her family were informed about emergency exploratory laparotomy procedure and relevant medical documents were signed after obtaining their consent.

Abstract:Objective To evaluate clinical manifestations,diagnosis and treatment of congenital duodenal obstruction (CDO) in neonates.Methods Retrospective analysis of 246 neonatal patients with CDO admitted between January 2002 and December 2011 was conducted.Clinical data including demographic criteria,clinical manifestations,associated anomalies,radiologic findings,operations,postoperative complications and final outcomes were recorded and analyzed.Total 246 newborns were enrolled in this study,with 173 boys and 73 girls.Birth weight (BW) ranged from 950 g to 4 850 g.Forty-three babies were born prematurely.One hundred and fifty eight cases were diagnosed with malrotation,49 cases with annular pancreas,45 cases with duodenal web,7 cases with duodenal atresia or stenosis and 1 case of duodenal abnormal bands,while 14 cases had various combinations of these conditions.One hundred and twenty four patients had associated anomalies.Results Operative procedures included Ladd's procedure,duodenoduodenostomy,excision of the web and duodenoplasty,duodenojejunostomy or combination of the above.Seventeen patients died or withdraw treatment postoperatively.The causes of death were combinations of prematurity,sepsis,pneumonia,pulmonary hemorrhage,short bowel,or cardiac anomaly.Fourteen patients were reoperated for wound dehiscence,adhesive intestinal obstruction,anastomotic leak,cholestasis,missed diagnosis or recurrence of malrotation.Enteral feeding was started at an average of 8.51 ± 3.17 days after operation,and the mean hospital stay was 14.87 ± 5.60 days.Conclusions CDO is a complex entity with various causations and multiple accompanied diseases.Timely diagnosis and aggressive surgery are key points for better prognosis.Multiple causes of duodenal obstruction and/or associated alimentary tract anomalies should always be aware during operation.
A report of 73 cases with primary duodenal tumors

Abstract:Objective To investigate the clinical features, pathogenesis and diagnostic and therapeutic methods of primary duodenal tumors.Method Clinical data of 73 patients with primary duodenal tumors who hospitalized in the Second Hospital Affiliated to Zhejiang University Medical School during the period of 1982～2000 were analyzed retrospectively. The average age of the patients was 54.7 years old and the average interval from onset of symptoms to treatment was 62 days. Of all the tumors, 25 cases were benign tumors whose pathological types included adenoma (20 cases), leiomyoma(3 cases) and adeomyoma(2 cases) and 48 cases were malignant tumors whose pathological types included adenocarcinoma(41 cases), leiomyosarcoma(4 cases), small cell undifferentiated carcinoma(1 case), cracinoid tumor(1 case) and leiomyoblastoma(1 case).Result Tumors mostly located in the descending portion of the duodenum (63 cases). The exophytic and local growth were observed in the benign tumors and the invasive growth were found in the malignant tumors. In addition, the metastasis of lymph nodes might be observed in the malignant tumors. The symptoms and signs of the bile duct obstruction, GI and nonspecific symptoms and signs including fever, anemia and weight loss were presented. Among the numerous diagnostic procedures, GI barium meal, gastroscopy and ERCP were especially useful and had more efficiency. 47 cases were treated with pancreatoduodenectomy including 5 cases treated with binding pancreaticojejunostomy. 3 cases of adenoma located in duodenal ampulla were removed by endoscopy. 12 cases of benign tumors underwent local resection and 2 cases underwent partial duodenum resection and most partial gastric resection. 9 cases of malignant tumors missed radical excision because of widespread or metastasis.Conclusion Most primary duodenal tumors often located in the descending portion of duodenum. Adenoma predominated among benign tumors and adenocarcinoma among malignant tumors. Patients usually lacked specific symptoms and signs during the early stage. Diagnostic procedures must be selected purposely and combined to use to improve confirmed diagnostic rate. Surgical treatment is still the chief therapeutics for primary duodenal tumors at present.
Primary Reconstruction of Bladder Neck Transection and Vaginal Laceration Post Blunt Abdominal Injury a Case Report

Abstract:Blunt abdominal injury is not uncommon in the paediatric population. The presentation can be immediate or delayed, and if not promptly recognized may be attended with complications such as septicaemia, shock or death. Simple, readily available and cheap imaging techniques such as abdominal radiograph and ultrasound scan still remain relevant in the diagnosis of blunt abdominal injuries where abdominal computerized tomography is unavailable or unaffordable. This can expedite the decision for an early surgical exploration. In girls, complete bladder neck transection with vaginal wall injury is a rarely reported and unusual association of blunt abdominal injury more so with delayed presentation in the absence of an associated pelvic fracture. This rare trauma entity in a child can pose a diagnostic difficulty leading to delayed presentation. This is a case of 3 year old girl with 3 weeks history of trauma to the abdomen from collapsed building fence during a heavy downpour. She later developed progressive abdominal distension with pain, constipation, vomiting, fever and drainage of clear fluid per vaginam. She had visited several hospitals but was managed non-operatively. Over the period the patient had some relief of symptoms until she presented to our facility with poorly defined features of peritonitis following a referral from a peripheral hospital. Abdominal ultrasound scan revealed intraperitoneal fluid collection. She was resuscitated and had exploratory laparotomy with primary reconstruction of the bladder neck transection and vaginal wall laceration. She did well immediate postoperative except for the superficial surgical site infection and during follow-up visits. Primary repair of traumatic bladder neck transection and vaginal wall laceration in paediatric patients with delayed presentation of blunt abdominal injury is a feasible option of management. However, long term follow up is recommended as secondary procedures may become necessary.
Traumatic Gallbladder Perforation in Children - Case Report and Review

Abstract:Background: Gallbladder perforation is a very rare finding in children. Clinical and radiografic presentations are often vague. Hence it is a challenging diagnosis, often missed during initial evaluation with potentially fatal consequences. The aim of this case report and review was to identify factors that might help to diagnose and manage future cases. Methods: We present a case of gallbladder perforation in an 8-year-old child after blunt abdominal trauma caused by a handlebar in which imaging modalities such as ultrasound and magnetic resonance imaging (MRI) helped us to determine proper management. We identified and evaluated comparable cases for isolated traumatic gallbladder injury. A review of the recent literature was performed by online search in Pubmed and Google Scholar using "gallbladder injury in children", "gallbladder perforation children", "blunt abdominal trauma children" as keywords. We focused on articles exclusively in the pediatric section. The literature from the period 2000-2020 was taken into account to review the state of the art in diagnosis and management. Results and conclusion: Recent literature for gallbladder injury in pediatrics is sparse compared to the adult counterpart. Only eight published cases of isolated gallbladder injury in children were identified. Since the onset of symptoms may not develop acutely and often develops in an indistinct manner, radiografic examinations play an important role in the diagnostic progress. The authors advise caution in cases of blunt abdominal trauma especially involving handlebars, intraperitoneal free fluid, and severe abdominal pain. We advocate high suspicion of gallbladder perforation if the gallbladder wall displays discontinuation or cannot be definitively differentiated in sonografic examination. Echogenic fluid within the gallbladder should always lead to suspicion of intraluminal bleeding. Repeated clinical and imaging examinations are mandatory since the onset of signs and symptoms may be delayed. Experience of management of pediatric upper gastrointestinal perforations: a series of 30 cases

Abstract:Background: This study aimed to explore the characteristics of pediatric upper gastrointestinal (UGI) perforations, focusing on their diagnosis and management. Methods: Between January 2013 and December 2021, 30 children with confirmed UGI perforations were enrolled, and their clinical data were analyzed. Two groups were compared according to management options, including open surgical repair (OSR) and laparoscopic/gastroscopic repair (LR). Results: A total of 30 patients with a median age of 36.0 months (1 day–17 years) were included in the study. There were 19 and 11 patients in the LR and OSR groups, respectively. In the LR group, two patients were treated via exploratory laparoscopy and OSR, and the other patients were managed via gastroscopic repair. Ten and three patients presented the duration from symptom onset to diagnosis within 24 h ( p = 0.177) and the number of patients with hemodynamically unstable perforations was 4 and 3 in the LR and OSR groups, respectively. Simple suture or clip closure was performed in 27 patients, and laparoscopically pedicled omental patch repair was performed in two patients. There was no significant difference in operative time and length of hospital stay between the LR and OSR groups. Treatment failed in two patients because of severe sepsis and multiple organ dysfunction syndrome, including one with fungal peritonitis. Conclusion: Surgery for pediatric UGI perforations should be selected according to the general status of the patient, age of the patient, duration from symptom onset, inflammation, and perforation site and size. Antibiotic administration and surgical closure remain the main strategies for pediatric UGI perforations.

Rupture of Main Bronchus after Blunt Chest Trauma in Children

Abstract:Objective To evaluate the clinical features of rupture of main bronchus after blunt chest trauma in children and the importance of early diagnosis and surgical treatment.Methods 11 consecutive patients from ages 3 to 7 years(5.1±1.4),with the main bronchial disruptions due to blunt chest trauma were reported(four right,seven left).8 boys and 3 girls were included.4 patients underwent immediate surgical correction (end-to-end anastomosis)within 24 hours after the accident.7 patients had a delayed diagnosis for 2-12 months after injury.Among these 7 patients,end￣to￣end anastomosis was used in 3 cases,four patients required total pneumonectomy.Results There were no deaths in this series.7 patients who received end-to-end anastomosis experienced an uneventful recovery.Conclusion Recognition of the clinical features of main bronchial rupture of main bronchus after blunt chest trauma in children,early diagnosis and bronchial repair could significantly decrease the incidence of complication and mortality.

Abstract:Purpose/background: Meckel's diverticulum (MD) is one of the most common congenital malformations of gastrointestinal tract in children. However, the nonspecific clinical manifestations of MD often cause a diagnostic as well as therapeutic challenge to pediatric surgeon. This study aimed to review our experience in managing this disease while evaluating the management strategies. Methods: We retrospectively analyzed the clinical data of all patients diagnosed with MD admitted to our center between January 2010 and December 2015. Factors documented including demographic criteria, clinical manifestations, preoperative examinations, surgical methods, histopathological characteristics, postoperative complications, and outcomes. Results: The patients included 210 males and 76 females, aged from 1 day to 15 years. In fifty three patients, the MD was an incidental finding at laparotomy or laparoscopy. The remaining 233 patients were symptomatic and presented with various clinical features. Ninety nine patients presented with episodes of bleeding per rectumor melena. Fifty six patients demonstrated symptoms of diverticulitis or perforated MD. Forty patients were diagnosed as intestinal obstruction, and 35 patients with intussusception requiring surgical reduction. Two cases of Littre hernia and one case of foreign body trapped in MD were also observed in this group. Six patients misdiagnosed as appendicitis at another institution were reoperated in our department. Among the 99 patients with bleeding per rectum, 78 underwent a Tc-99mscan that showed a positive tracer in 55 patients and negative in 23. All patients underwent resection of the diverticulum, except for 2 cases of postponed resection. Histology revealed ectopic gastric mucosa or ectopic pancreatic tissue in 154 patients; significant differences were observed between the symptomatic group and the accidentally found group. One patient died of peritonitis and sepsis postoperatively; one case of anastomotic leak and one case of adhesive intestinal obstruction were reoperated. Conclusion: Meckel's diverticulumhas various clinical presentations and it is difficult to make a precise diagnosis preoperatively. It is necessary to maintain a high suspicion of MD in the pediatric age group with symptoms of abdominal pain, gastrointestinal hemorrhage or intestinal obstruction. Heterotopic tissue is the main cause of complicated diverticulum, and it is safe and feasible to remove the incidentally found MD. Laparoscopy should become the first choice of methods in diagnosis and treatment of MD. Type of study: Treatment study. (c) 2017 Published by Elsevier Inc.

Abstract:The presence of incomplete upper gastrointestinal obstruction in childhood is a rare condition and usually indicates congenital anomaly. We reported a case of incomplete duodenal obstruction in a 16-year-old girl due to a congenital duodenal diaphragm. A duodenal diaphragm with an eccentric aperture was found by upper gastrointestinal series and endoscopic examination in the second part of the duodenum. The patient made an uncomplicated recovery with relief of the symptoms after duodenotomy and excision of the diaphragm. This case underlines that congenital duodenal diaphragm should be kept in mind as a possibility whenever chronic obstruction of the duodenum in childhood is diagnosed. The prognosis of congenital duodenal diaphragm is usually good after excision of the diaphragm, when not associated with other congenital anomalies.
