Pediatric goal-directed perfusion: is there a general target?.Ann Thorac Surg. 2022; 113: 1052-1053Google Scholar whether the nadir indexed oxygen delivery (DO2i) of 353 mL/min/m2 can be a general target for pediatric goal-directed perfusion (GDP), we note that there are numerous factors during cardiopulmonary bypass (CPB) such as temperature, metabolic rate, hemoglobin (Hb), surgical complexity can affect global oxygen delivery. Therefore, a general target tailored to all pediatric patients in different situations is hard to achieve. However, we can explore the DO2i threshold suitable for certain situation. We completely agree that there is significant heterogeneity in pediatric patients. Pediatric patients (age≦7 years) can be divided into three subgroups: neonates, infants, preschool children according to their different physiological characteristics. We boldly guess that different limits of critical DO2i are likely to occur in different cohorts. Given that most pediatric patients undergoing cardiac surgery in our hospital choose to have surgery at their earlier age and infants have similar body surface area (BSA) and metabolic rate, this age demographic (1 month ≦ age ≦ 3 years) was chosen in our study. And now, we are also carrying out a trial to further explore the critical thresholds of DO2i in the sub-population of neonates and preschool children, respectively. It is true that every hospital has its own habit about the management of transfusion and flow rate during CPB. Patients with nadir CPB temperature < 32°C were excluded in our study. Therefore, majority operations for severe cyanosis cases have been excluded. This largely avoids the bias to some extent caused by temperature and operative type. Furthermore, although DO2i cannot reflect the difference in high flow rate with low hematocrit compared to low flow rate with high hematocrit, it is more comprehensive in reflecting tissue oxygen supply than the independent parameters of arterial oxygen saturation (SaO2), Hb concentration and flow rate.3Murphy G.S. I believe our critical DO2i threshold could specifically tailored to the infants undergoing cardiac surgery with mild hypothermia CPB. In the future, further prospective, large-sample, multicenter studies are required to explore the clinical thresholds of various GDP monitoring indicators such as oxygen consumption (VO2), indexed carbon dioxide production (VCO2i) and their effects of practical application, so as to form consensus and guidelines.
