Expert Opinion on Therapeutic Targets

Abstract:ABSTRACT Introduction Pre-eclampsia is a serious pregnancy complication and a major global concern for the mortality of both mother and fetus. Existing symptomatic treatments do not delay disease progression; thus, timely delivery of the baby is the most effective measure. However, the risk of various maternal and fetal injuries remains. Areas covered In this review, we summarize potential strategies for pharmacologic interventions in pre-eclamptic therapy. Specifically, we discuss the pathophysiological process of various effective candidate therapeutics that act on potential pathways and molecular targets to inhibit key stages of the disease. We refer to this pathogenesis-focused drug discovery model as a pathogenesis–target–drug (P–T–D) strategy. Finally, we discuss the introduction of nanotechnologies to improve the safety and efficacy of therapeutics via their specific placental targeting ability and placental retention effects. Expert opinion Despite the active development of novel pharmacological treatments based on our current knowledge of pre-eclamptic pathogenesis, investigations are still in the early phase. Thus, further exploration of the pathological mechanisms, integrated with the P–T–D strategy and novel nanosystems, could encourage more effective and safer strategies. Such advances could lead to a shift from expectant management to mechanistic-based therapy for pre-eclampsia.

Abstract:Pre-eclampsia is a complication of pregnancy characterized by hypertension and proteinuria that results in maternal and neonatal mortality and morbidity.Although the etiology remains unclear,it seems to be initiated by placental factors that enter the maternal circulation and may cause endothelial dysfunction.Recently,anti-angiogenic factors such as endoglin,the soluble form of vascular endothelial growth factor receptor(sFlt-1) as well as the placental growth factor have been found to be associated with the onset of pre-eclampsia.This review will summarize our current understanding of molecular structure of sFlt-1,signal pathways,with an emphasis on its role in the pathogenesis of preeclampsia and regulative factors.

Abstract:Preeclampsia is a common pregnancy complication that causes maternal and fetal morbidity and mortality.Although the incidence is higher in nulliparous women than in multiparae,any multipara who has a history of PE bears a higher risk,and if the previous PE is of an early onset,or complicated with eclampsia,HELLP syndrome or FGR,the recurrence risk goes even higher,probably with an even severer degree.The study of the reccurent PE is very challenging for obstetricians. For the multiparous women, the obstetricians should focus on providing a better antenatal counselling,promoting the mothers' health, standardizing the prenatal examination,taking comprehensive preventive measures,monitoring the gestational process and delivery timing,so as to maximize the interests of both the mothers and the babies.

Abstract:Objective Pre-eclampsia is a common disease during pregnancy that leads to fetal and maternal adverse events. Few head-to-head clinical trials are currently comparing the effectiveness of prophylactic strategies for pre-eclampsia. In this network meta-analysis, we aimed to compare the efficacy of prophylactic strategies for preventing pre-eclampsia in pregnant women at risk. Data Sources Articles published in or before September 2021 from PubMed, EMBASE, Web of Science, Cochrane Library, and Clinicaltrials.gov, references of key articles and previous meta-analyses were manually searched. Study eligibility criteria Randomized controlled trials comparing prophylactic strategies preventing pre-eclampsia with each other or with negative controls. Study appraisal and synthesis methods : Two reviewers independently extracted data, assessed the risk of bias, and assessed evidence certainty. The efficacy of prophylactic strategies was estimated by frequentist and Bayesian network meta-analysis models. The composite primary outcome was pre-eclampsia/ pregnancy-induced hypertension. Results In total, 130 trials with a total of 112,916 patients were included to assess 13 prophylactic strategies. Low molecular weight heparin (LMWH) (0.60, 95% CI 0.42-0.87), vitamin D supplementation (0.65, 95% CI 0.45-0.95) and exercise (0.68, 95% CI 0.50-0.92) were as efficacious as aspirin (0.79, 95% CI 0.72-0.86) and calcium supplementation (0.71, 95% CI 0.62-0.82) in preventing pre-eclampsia/pregnancy-induced hypertension, which was ranked by P-Score: LMWH (85%), vitamin D (79%), exercise (76%), calcium (74%) and aspirin (61%). In the head-to-head comparison, no differences were found between those effective prophylactic strategies for preventing pre-eclampsia and pregnancy-induced hypertension, except for exercise tended to be superior to aspirin and calcium supplementation with respect to preventing pregnancy-induced hypertension. Furthermore, the prophylactic effects of aspirin and calcium supplementation were robust across subgroups. However, the prophylactic effects of LMWH, exercise, and vitamin D supplementation on pre-eclampsia and pregnancy-induced hypertension varied with the different risk populations, dosages, areas, and forth. The certainty of the evidence was moderate to very low. Conclusions LMWH, vitamin D supplementation, exercise, calcium supplementation, and aspirin reduce the risk of pre-eclampsia/pregnancy-induced hypertension. No significant differences between effective prophylactic strategies were found in preventing pre-eclampsia. These findings raise the necessity to reevaluate the prophylactic effects of LMWH, vitamin D supplementation and exercise on pre-eclampsia.

Revisiting preeclampsia: a metabolic disorder of the placenta

Abstract:Preeclampsia (PE) is a leading cause of maternal and neonatal mortality and morbidity worldwide, impacting the long-term health of both mother and offspring. PE has long been characterized by deficient trophoblast invasion into the uterus and consequent placental hypoperfusion, yet the upstream causative factors and effective interventional targets for PE remain unknown. Alterations in the metabolism of preeclamptic placentas are thought to result from placental ischemia, while disturbances of the metabolism and of metabolites in PE pathogenesis are largely ignored. In fact, as one of the largest fetal organs at birth, the placenta consumes a considerable amount of glucose and fatty acid. Increasing evidence suggests glucose and fatty acid exist as energy substrates and regulate placental development through bioactive derivates. Moreover, recent findings have revealed that the placental metabolism adapts readily to environmental changes, altering its response to nutrients and endocrine signals; this adaptability optimizes pregnancy outcomes by diversifying available carbon sources for energy production, hormone synthesis, angiogenesis, immune activation, and tolerance, and fetoplacental growth. These observations raise the possibility that carbohydrate and lipid metabolism abnormalities play a role in both the etiology and clinical progression of PE, sparking a renewed interest in the interrelationship between PE and metabolic dysregulation. This review will focus on key metabolic substrates and regulatory molecules in the placenta and aim to provide novel insights with respect to the metabolism's role in modulating placental development and functions. Further investigations from this perspective are poised to decipher the etiology of PE and suggest potential therapies.

Abstract:Hypertension is the most common medical disorder encountered during pregnancy. Hypertensive disorders are one of the major causes of pregnancy-related maternal deaths in the United States. We will present a comprehensive update of the literature pertinent to hypertension in pregnancy. The paper begins by defining and classifying hypertensive disorders in pregnancy. The normal vascular and renal physiological changes which occur during pregnancy are detailed. We will summarize the intriguing aspects of pathophysiology of preeclampsia, emphasizing on recent advances in this field. The existing diagnostic tools and the tests which have been proposed for screening preeclampsia are comprehensively described. We also highlight the short- and long-term implications of preeclampsia. Finally, we review the current management guidelines, goals of treatment and describe the potential risks and benefits associated with various antihypertensive drug classes. Preeclampsia still remains an enigma, and the present management focuses on monitoring and treatment of its manifestations. We are hopeful that this in depth critique will stimulate the blossoming research in the field and assist practitioners to identify women at risk and more effectively treat affected individuals.

Abstract:Objective:To investigate the impact of preeclampsia(PE) and risk factors for perinatal outcomes.Methods:According to research status at home and abroad,a random investigation of 154 pregnant women with or without high-risk factors,who did pre-pregnancy physical examinations and deliveries from Apr.2011 to Jan.2012 at Renji Hospital,Medical School of Shanghai Jiaotong University.Finally,there were 50 cases of PE and 104 cases of non-PE.Perinatal outcomes in preeclampsia and risk factors were analyzed by tracking pregnancy outcomes with acquisition of medical history and laboratory tests.Results:(1)Neonatal weight and gestational age of the preeclampsia group was significantly less than that of the non-preeclampsia group.(2)The multiple of incidence rate in preeclampsia group was 4.64 times,2.32 times,4.61 times,2.66 times and 6.38 times,respectively,compared to non-preeclampsia group in fetal growth restriction(FGR),fetal distress,premature delivery,stillbirth and neonatal asphyxia,while the incidence of FGR,fetal distress and preterm birth were statistically significant higher(P<0.05).(3)Neonatal asphyxia,FGR,fetal distress and perinatal death rate increased significantly when MAP≥85mmHg,proteinuria,abnormal uterine/umbilical artery blood flow,DD>0.246μg/ml,FDP>4μg/ml,PAGT>40%.Different risk factors threatened perinatal outcome differently while the abnormal uterine/umbilical artery blood flow threatened most(P<0.05).Conclusions:Preeclampsia can lead to adverse perinatal outcomes.It may help to prevent PE and perinatal adverse outcomes with strict screening for high-risk pregnant women and close monitoring of the growth.

Abstract:Preeclampsia (PE) is a life-threatening disease of pregnancy and a prominent cause of neonatal and maternal mortality and morbidity. PE affects approximately 5-10% of pregnancies worldwide, posing significant risks to perinatal and maternal health. It is characterized by a variety of interconnected pathological cascades contributing to the stimulation of intravascular inflammation, oxidative stress (OS), endothelial cell activation, and syncytiotrophoblast stress that converge on a common pathway, ultimately resulting in disease progression. The present study was designed and executed to review the existing scientific literature, specifically focusing on the etiology (gestational diabetes mellitus and maternal obesity, insulin resistance, metabolic syndrome, maternal infection, periodontal disease, altered microbiome, and genetics), clinical presentations (hypertension, blood disorders, proteinuria, hepatic dysfunction, renal dysfunction, pulmonary edema, cardiac dysfunction, fetal growth restrictions, and eclampsia), therapeutic clinical biomarkers (creatinine, albuminuria, and cystatin C) along with their associations and mechanisms in PE. In addition, this study provides insights into the potential of nanomedicines for targeting these mechanisms for PE management and treatment. Inflammation, OS, proteinuria, and an altered microbiome are prominent biomarkers associated with progression and PE-related pathogenesis. Understanding the molecular mechanisms, exploring suitable markers, targeted interventions, comprehensive screening, and holistic strategies are critical to decreasing the incidence of PE and promoting maternal-fetal well-being. The present study comprehensively reviewed the etiology, clinical presentations, therapeutic biomarkers, and preventive potential of nanomedicines in the treatment and management of PE.

Abstract:Preeclampsia is a hypertensive disorder of pregnancy. It affects 2% to 8% of pregnancies worldwide and causes significant maternal and perinatal morbidity and mortality. Hypertension and proteinuria are the cornerstone of the disease, though systemic organ dysfunction may ensue. The clinical syndrome begins with abnormal placentation with subsequent release of antiangiogenic markers, mediated primarily by soluble fms-like tyrosine kinase-1 (sFlt-1) and soluble endoglin (sEng). High levels of sFlt-1 and sEng result in endothelial dysfunction, vasoconstriction, and immune dysregulation, which can negatively impact every maternal organ system and the fetus. This review comprehensively examines the pathogenesis of preeclampsia with a specific focus on the mechanisms underlying the clinical features. Delivery is the only definitive treatment. Low-dose aspirin is recommended for prophylaxis in high-risk populations. Other treatment options are limited. Additional research is needed to clarify the pathophysiology, and thus, identify potential therapeutic targets for improved treatment and, ultimately, outcomes of this prevalent disease.
Different Proteomic Profiles Regarding Antihypertensive Therapy in Preeclampsia Pregnant

Abstract:Preeclampsia (PE) is a hypertensive pregnancy syndrome associated with target organ damage and increased cardiovascular risks, necessitating antihypertensive therapy. However, approximately 40% of patients are nonresponsive to treatment, which results in worse clinical outcomes. This study aimed to compare circulating proteomic profiles and identify differentially expressed proteins among 10 responsive (R-PE), 10 nonresponsive (NR-PE) patients, and 10 healthy pregnant controls (HP). We also explored correlations between these proteins and clinical data. Plasma protein relative quantification was performed using mass spectrometry, followed by bioinformatics analyses with the UniProt database, PatternLab for Proteomics 4.0, and MetaboAnalyst software (version 6.0). Considering a fold change of 1.5, four proteins were differentially expressed between NR-PE and R-PE: one upregulated (fibronectin) and three downregulated (pregnancy-specific beta-1-glycoprotein 1, complement C4B, and complement C4A). Between NR-PE and HP, six proteins were differentially expressed: two upregulated (clusterin and plasmin heavy chain A) and four downregulated (apolipoprotein L1, heparin cofactor II, complement C4B, and haptoglobin-related protein). Three proteins were differentially expressed between R-PE and HP: one downregulated (transthyretin) and two upregulated (apolipoprotein C1 and hemoglobin subunit beta). These findings suggest a complex interplay of these proteins involved in inflammatory, immune, and metabolic processes with antihypertensive therapy responsiveness and PE pathophysiology.

Abstract:This review reflects both the variable presentation and the systemic nature of preeclampsia. Recommendations for the comprehensive evaluation and management of organ dysfunction associated with pre-eclampsia are included. The main points in the review are that: (1) Preeclampsia is a systemic disorder that may affect many organ systems. (2) For preeclampsia remote from term (<34 weeks), expectant management improves perinatal outcomes, but requires obsessive surveillance to mitigate maternal risks and is a "package." (3) Initial assessment and ongoing surveillance of women with preeclampsia should include assessment of all vulnerable maternal organs as well as of the fetus. (4) Initiate antihypertensive drug treatment immediately if sBP >160 mmHg or dBP more than 110 mmHg, or if sBP 140-159 mmHg and/or dBP 85-109 mmHg (prepregnancy renal disease or diabetes). (5) The treatment of nonsevere pregnancy hypertension should include a treatment goal of dBP 80-105 mmHg (depending on practitioner preference), with one of the following agents, Methyldopa, Labetalol, Nifedipine, or, with special indications (renal or cardiac diseases), diuretics. (6) Drugs to avoid: angiotensin-converting enzyme inhibitors; angiotensin II receptor antagonists; and atenolol. (7) For the acute management of severe hypertension, initially reduce dBP by 10 mmHg and maintain the blood pressure at or below that level with either Nifedipine or Labetalol. (8) For both prophylaxis against and treatment of eclampsia, MgSO4 (4 g IV stat, then 1 g/hr). (9) For recurrent seizures, MgSO4 (2g IV stat, then increase to 1.5 g/hr). (10) Total fluid intake should not exceed 80 ml/hr; tolerate urine outputs as low as 10 ml/hr. (11) Early-onset and/or severe preeclampsia predict later cardiovascular morbidity and mortality; it would seem prudent to offer such women screening and lipid lowering interventions.

Abstract:Purpose: Preeclampsia is a major cause of health problems for both pregnant women and unborn babies worldwide. However, the underlying causes of preeclampsia are not fully understood, leading to limited effective treatments. The goal of this study is to enhance our knowledge of its causes, devise prevention strategies, and develop treatments. Methods: We performed a systematic literature search. Six models regarding the pathogenesis of preeclampsia are discussed in this review. Results: This review focuses on the latest advancements in understanding preeclampsia's origins. Preeclampsia is a complex condition caused by various factors, processes, and pathways. Reduced blood flow and oxygen to the uterus and placenta, heightened inflammatory reactions, immune imbalances, altered genetic changes, imbalanced blood vessel growth factors, and disrupted gut bacteria may contribute to its development. Conclusion: Preeclampsia is thought to result from the interplay of these factors.
Preeclampsia: an update

Abstract:Preeclampsia was formerly defined as a multisystemic disorder characterized by new onset of hypertension (i.e. systolic blood pressure (SBP) ≥ 140 mmHg and/or diastolic blood pressure (DBP) ≥ 90 mmHg) and proteinuria (> 300 mg/24 h) arising after 20 weeks of gestation in a previously normotensive woman. Recently, the American College of Obstetricians and Gynecologists has stated that proteinuria is no longer required for the diagnosis of preeclampsia. This complication of pregnancy remains a leading cause of maternal morbidity and mortality. Clinical signs appear in the second half of pregnancy, but initial pathogenic mechanisms arise much earlier. The cytotrophoblast fails to remodel spiral arteries, leading to hypoperfusion and ischemia of the placenta. The fetal consequence is growth restriction. On the maternal side, the ischemic placenta releases factors that provoke a generalized maternal endothelial dysfunction. The endothelial dysfunction is in turn responsible for the symptoms and complications of preeclampsia. These include hypertension, proteinuria, renal impairment, thrombocytopenia, epigastric pain, liver dysfunction, hemolysis-elevated liver enzymes-low platelet count (HELLP) syndrome, visual disturbances, headache, and seizures. Despite a better understanding of preeclampsia pathophysiology and maternal hemodynamic alterations during preeclampsia, the only curative treatment remains placenta and fetus delivery. At the time of diagnosis, the initial objective is the assessment of disease severity. Severe hypertension (SBP ≥ 160 mm Hg and/or DBP ≥ 110 mmHg), thrombocytopenia < 100.000/μL, liver transaminases above twice the normal values, HELLP syndrome, renal failure, persistent epigastric or right upper quadrant pain, visual or neurologic symptoms, and acute pulmonary edema are all severity criteria. Medical treatment depends on the severity of preeclampsia, and relies on antihypertensive medications and magnesium sulfate. Medical treatment does not alter the course of the disease, but aims at preventing the occurrence of intracranial hemorrhages and seizures. The decision of terminating pregnancy and perform delivery is based on gestational age, maternal and fetal conditions, and severity of preeclampsia. Delivery is proposed for patients with preeclampsia without severe features after 37 weeks of gestation and in case of severe preeclampsia after 34 weeks of gestation. Between 24 and 34 weeks of gestation, conservative management of severe preeclampsia may be considered in selected patients. Antenatal corticosteroids should be administered to less than 34 gestation week preeclamptic women to promote fetal lung maturity. Termination of pregnancy should be discussed if severe preeclampsia occurs before 24 weeks of gestation. Maternal end organ dysfunction and non-reassuring tests of fetal well-being are indications for delivery at any gestational age. Neuraxial analgesia and anesthesia are, in the absence of thrombocytopenia, strongly considered as first line anesthetic techniques in preeclamptic patients. Airway edema and tracheal intubation-induced elevation in blood pressure are important issues of general anesthesia in those patients. The major adverse outcomes associated with preeclampsia are related to maternal central nervous system hemorrhage, hepatic rupture, and renal failure. Preeclampsia is also a risk factor for developing cardiovascular disease later in life, and therefore mandates long-term follow-up.

Abstract:Objectives: To review, evaluate, and synthesize the recommendations of guidelines on preeclampsia (PE) from a nursing perspective. Methods: This is a systematic review of international and national guidelines on PE. Electronic databases and related guideline websites were searched from 2013 to 2023. After systematic retrieval and screening, we used the AGREE II tool to appraise the methodological quality of guidelines that met the eligibility criteria. Then, we analyzed and summarized the recommendations using descriptive analysis and the framework method. Furthermore, we rated the quality of evidence and the strength of the recommendations using the GRADE approach. Results: Ten guidelines were included, among which eight were deemed “clinically useful” and records were extracted. In total, 31 recommendations, including 46 items on the nursing management of PE, were summarized from three aspects: 1) antenatal care (18 recommendations), including the assessment and regular monitoring of PE, standardized blood pressure (BP) measurement, prevention education for PE, antihypertensive agent monitoring, and magnesium sulfate (MgSO4) use; 2) intrapartum care (4 recommendations), including childbirth care; 3) postpartum care (9 recommendations), including regular nursing monitoring, breastfeeding care and longer-term health counseling. Most of the evidence was rated as “very low” (19/46) or “moderate” (15/46) quality. For the strength of the recommendations, 30 items were graded as “strong” and 16 items were rated as “weak”. Conclusions: This study provides professional, evidence-based nursing care resources both for preservice education for nursing staff and health care education for women with PE to help them detect and treat PE in a timely manner.
Gestational Hypertension and Preeclampsia: An Overview of National and International Guidelines

Abstract:IMPORTANCE: Gestational hypertension and preeclampsia are leading causes of maternal and perinatal morbidity and mortality worldwide. Τhe lack of effective screening and management policies appears to be one of the main reasons.OBJECTIVE: The aim of this study was to review and compare recommendations from published guidelines on these common pregnancy complications.EVIDENCE ACQUISITION: A descriptive review of guidelines from the National Institute for Health and Care Excellence, the Society of Obstetric Medicine of Australia and New Zealand, the International Society of Hypertension, the International Society for the Study of Hypertension in Pregnancy, the European Society of Cardiology, the International Federation of Gynecology and Obstetrics, the Society of Obstetricians and Gynaecologists of Canada, the American College of Obstetricians and Gynecologists, the International Society of Ultrasound in Obstetrics and Gynecology, the World Health Organization, and the US Preventive Services Task Force on gestational hypertension and preeclampsia was carried out.RESULTS: There is an overall agreement that, in case of suspected preeclampsia or new-onset hypertension, blood and urine tests should be carried out, including dipstick test for proteinuria, whereas placental growth factor-based testing is only recommended by the National Institute for Health and Care Excellence and the European Society of Cardiology. In addition, there is a consensus on the recommendations for the medical treatment of severe and nonsevere hypertension, the management of preeclampsia, the appropriate timing of delivery, the optimal method of anesthesia and the mode of delivery, the administration of antenatal corticosteroids and the use of magnesium sulfate for the treatment of eclamptic seizures, the prevention of eclampsia in cases of severe preeclampsia, and the neuroprotection of preterm neonates. The reviewed guidelines also state that, based on maternal risk factors, pregnant women identified to be at high risk for preeclampsia should receive low-dose aspirin starting ideally in the first trimester until labor or 36 to 37 weeks of gestation, although the recommended dose varies between 75 and 162 mg/d. Moreover, most guidelines recommend calcium supplementation for the prevention of preeclampsia and discourage the use of other agents. However, controversy exists regarding the definition and the optimal screening method for preeclampsia, the need for treating mild hypertension, the blood pressure treatment targets, and the postnatal blood pressure monitoring.CONCLUSIONS: The development and implementation of consistent international protocols will allow clinicians to adopt effective universal screening, as well as preventive and management strategies with the intention of improving maternal and neonatal outcomes.

Abstract:Preeclampsia (PE) is one of the most important human pregnancy complications, for which the pathophysiology is poorly defined. There are no validated and consensual animal models that resemble disease process in women; the mechanism underlying the increasing cardiovascular diseases (CVD) risk in mother and offspring by PE remains unclear. The aim of this study is to establish and characterize a Reduced Uterine Perfusion Pressure (RUPP) model in rats with characteristic features of PE and to use it for therapeutical trial and offspring health programming study. For this purpose, pregnant SD rats underwent a placental ischemia induction, based on timing and positional ligation of vessels. After RUPP operation, mean arterial pressure and proteinuria significantly increased. At the same time, the levels of glutamate, proinflammatory cytokines, phosphorylated IKKβ, nuclear factor-kappa B (NF-κB) activity, Fra-LI (an indicator of chronic neuronal activation), superoxide, gp91phox, Ang II, AT1, and ACE(angiotensin converting enzyme) were significantly increased in hypothalamic paraventricularnucleus (PVN). Pyridostigmine, a reversible cholinesterase inhibitor, treatment attenuated hypertension, and proteinuria and significantly reduced cytokine, NF-κBp65 activity, reactive oxygen species levels, and altered renin-angiotensin system components in the PVN of RUPP rats. Our findings suggest that PE increases NF-κB activity, proinflammatory cytokine levels, and oxidative stress, inducing an imbalance between excitatory and inhibitory neurotransmitters in the PVN. PYR treatment attenuates BP and proteinuria by attenuating oxidative stress and modulating the balance of the autonomic nerve system and renin-angiotensin system in the hypothalamic paraventricularnucleus.
Preeclampsia: Recent Advances in Predicting, Preventing, and Managing the Maternal and Fetal Life-Threatening Condition

Abstract:Preeclampsia accounts for one of the most common documented gestational complications, with a prevalence of approximately 2 to 15% of all pregnancies. Defined as gestational hypertension after 20 weeks of pregnancy and coexisting proteinuria or generalized edema, and certain forms of organ damage, it is life-threatening for both the mother and the fetus, in terms of increasing the rate of mortality and morbidity. Preeclamptic pregnancies are strongly associated with significantly higher medical costs. The maternal costs are related to the extra utility of the healthcare system, more resources used during hospitalization, and likely more surgical spending due to an elevated rate of cesarean deliveries. The infant costs also contribute to a large percentage of the expenses as the babies are prone to preterm deliveries and relevant or causative adverse events. Preeclampsia imposes a considerable financial burden on our societies. It is important for healthcare providers and policy-makers to recognize this phenomenon and allocate enough economic budgets and medical and social resources accordingly. The true cellular and molecular mechanisms underlying preeclampsia remain largely unexplained, which is assumed to be a two-stage process of impaired uteroplacental perfusion with or without prior defective trophoblast invasion (stage 1), followed by general endothelial dysfunction and vascular inflammation that lead to systemic organ damages (stage 2). Risk factors for preeclampsia including race, advanced maternal age, obesity, nulliparity, multi-fetal pregnancy, and co-existing medical disorders, can serve as warnings or markers that call for enhanced surveillance of maternal and fetal well-being. Doppler ultrasonography and biomarkers including the mean arterial pressure (MAP), uterine artery pulsatility index (UtA-PI), and serum pregnancy-associated plasma protein A (PAPP-A) can be used for the prediction of preeclampsia. For women perceived as high-risk individuals for developing preeclampsia, the administration of low-dose aspirin on a daily basis since early pregnancy has proven to be the most effective way to prevent preeclampsia. For preeclamptic females, relevant information, counseling, and suggestions should be provided to facilitate timely intervention or specialty referral. In pregnancies complicated with preeclampsia, closer monitoring and antepartum surveillance including the Doppler ultrasound blood flow study, biophysical profile, non-stress test, and oxytocin challenge test can be arranged. If the results are unfavorable, early intervention and aggressive therapy should be considered. Affected females should have access to higher levels of obstetric units and neonatal institutes. Before, during, and after delivery, monitoring and preparation should be intensified for affected gravidas to avoid serious complications of preeclampsia. In severe cases, delivery of the fetus and the placenta is the ultimate solution to treat preeclampsia. The current review is a summary of recent advances regarding the knowledge of preeclampsia. However, the detailed etiology, pathophysiology, and effect of preeclampsia seem complicated, and further research to address the primary etiology and pathophysiology underlying the clinical manifestations and outcomes is warranted.
