Abstract:AIM: To apply pre-processing and spectral analysis of systolic blood pressure and heart rate signals in conscious rats to evaluate changes in cardiovascular regulatory function induced by simulated weightlessness. METHODS: The tail-suspended, hindlimb-unloaded (HU) rat model was used to simulate the cardiovascular effect of microgravity. A pressure transducer connected to a PE-50-PE-10 catheter was inserted via the right femoral artery into the posterior abdominal aorta in conscious rats. We applied first derivative and template match algorithm to obtain systolic blood pressure variability (SBPV) data form original blood pressure data. PP time series extracted form the SBP time series were used to substitute the data of RR time series. SBPV data were analyzed by periodogram. RESULTS: An algorithm of first derivative and template match was suitable for effectively identifying SBP time series. The PP time series could replace RR time series. Power spectrum of SBPV, estimated by periodogram could be divided into 3 different frequency bands, very low frequency, low frequency, and high frequency. CONCLUSION: Periodogram is a useful method for blood pressure data processing, which is effective in detecting cardiovascular dysfunction in conscious rats after a 14-d simulated microgravity, and may be helpful for cardiovascular signal analysis in conscious rats.

Abstract:The disappearance of hydrostatic pressure is the original factor that causes the changes of cardiovascular system under microgravity. The hydrostatical changes can be simulated by postural changes. Especially the head-down position can be used to simulate the effects of microgravity. The goal of this investigation was to develop a mathematical model for simulation of the human cardiovascular responses to acute and prolonged exposure under microgravity environment. We were particularly interested in the redistribution of transmural pressures, flows, blood volume, and the consequent alterations in local hemodynamics in different cardiovascular compartments during acute exposure and chronic adjustments. As a preliminary study, we first developed a multi-element, distributed hemodynamic model of human cardiovascular system, and verified the model to simulate cardiovascular changes during head up/down tilt at various angles.

Abstract:In this study we demonstrate what kind of relative alterations can be expected in average perfusion and blood flow oscillations during postural changes being measured in the skin of limbs and on the brow of the forehead by wearable laser Doppler flowmetry (LDF) sensors. The aims of the study were to evaluate the dynamics of cutaneous blood perfusion and the regulatory mechanisms of blood microcirculation in the areas of interest, and evaluate the possible significance of those effects for the diagnostics based on blood perfusion monitoring. The study involved 10 conditionally healthy volunteers (44 ± 12 years). Wearable laser Doppler flowmetry monitors were fixed at six points on the body: two devices were fixed on the forehead, on the brow; two were on the distal thirds of the right and left forearms; and two were on the distal thirds of the right and left lower legs. The protocol was used to record three body positions on the tilt table for orthostatic test for each volunteer in the following sequence: (a) supine body position; (b) upright body position (+75°); (c) tilted with the feet elevated above the head and the inclination of body axis of 15° (−15°, Trendelenburg position). Skin blood perfusion was recorded for 10 min in each body position, followed by the amplitude–frequency analysis of the registered signals using wavelet decomposition. The measurements were supplemented with the blood pressure and heart rate for every body position analysed. The results identified a statistically significant transformation in microcirculation parameters of the average level of skin blood perfusion and oscillations of amplitudes of neurogenic, myogenic and cardiac sensors caused by the postural changes. In paper, we present the analysis of microcirculation in the skin of the forehead, which for the first time was carried out in various positions of the body. The area is supplied by the internal carotid artery system and can be of particular interest for evaluation of the sufficiency of blood supply for the brain.

Study on the Variability of Pulse Wave and ECG of Graduate Students with Different Body Positions

Abstract:Aims: To study the variations of pulse and ECG with different body positions. Methods: A device with a pressure-adjustable pulse sensor and an ECG sensor is used to acquire radial pulse wave and ECG signals, and a cuff sphygmomanometer is used to measure the blood pressure (BP). The signals are recorded continuously at different positions (supine, a series of transitional positions from supine to standing and standing position) for 20 subjects with age 24 ± 3(11 males, 9 females). Results: It is observed that the pulse contours of 20 subjects have two kinds of waveforms: one group has a tidal wave, while the other does not. In Group 1, the time that the valley of tidal wave occurred has no significant difference from supine to standing. The change of left ventricular ejection time (LVET) in Group 2 is larger than that in Group 1. During the procedure from supine to standing, the diastole period of the 95% subjects in two groups gets shorter, and all subjects' heart rate (HR) gradually accelerated. Meanwhile, pulse wave transit time (PWTT) decreases from supine to standing. In supine position, 100% subjects' diastole blood pressure (DBP) significantly decreased about 11.2 mmHg and 90% subjects' systolic blood pressure (SBP) reduced about 7.7 mmHg, which is consistent with the references. Conclusions: The experimental results demonstrate the diastole gets longer in supine position. The longer diastole helps the heart to get more rest time. Furthermore, blood pressure (BP) significantly decreased in supine which benefits cardiovascular system. Therefore, the supine position is a relatively ideal position in daily examining and clinical monitoring.
Critical analysis of autoregressive and fast Fourier transform markers of cardiovascular variability in rats and humans

Abstract:The autonomic nervous system plays an important role in physiological and pathological conditions, and has been extensively evaluated by parametric and non-parametric spectral analysis. To compare the results obtained with fast Fourier transform (FFT) and the autoregressive (AR) method, we performed a comprehensive comparative study using data from humans and rats during pharmacological blockade (in rats), a postural test (in humans), and in the hypertensive state (in both humans and rats). Although postural hypotension in humans induced an increase in normalized low-frequency (LFnu) of systolic blood pressure, the increase in the ratio was detected only by AR. In rats, AR and FFT analysis did not agree for LFnu and high frequency (HFnu) under basal conditions and after vagal blockade. The increase in the LF/HF ratio of the pulse interval, induced by methylatropine, was detected only by FFT. In hypertensive patients, changes in LF and HF for systolic blood pressure were observed only by AR; FFT was able to detect the reduction in both blood pressure variance and total power. In hypertensive rats, AR presented different values of variance and total power for systolic blood pressure. Moreover, AR and FFT presented discordant results for LF, LFnu, HF, LF/HF ratio, and total power for pulse interval. We provide evidence for disagreement in 23% of the indices of blood pressure and heart rate variability in humans and 67% discordance in rats when these variables are evaluated by AR and FFT under physiological and pathological conditions. The overall disagreement between AR and FFT in this study was 43%.

Abstract:We describe the multiresolution wavelet analysis of blood pressure waves in vasovagal syncope-affected patients compared with those in healthy people, using Haar and Gaussian bases. A comparison between scale-dependent and scale-independent measures discriminating the two classes of subjects is made. What emerges is a sort of equivalence between these two methodological approaches, that is, both methods reach the same statistical significance of separation between the two classes.

Abstract:Understanding cardiovascular control mediated by the autonomic system remains challenging due to its inherent complexity. Consequently, syndromes such as orthostatic intolerance continue to evoke debates regarding the underlying pathophysiological mechanisms. This study develops a comprehensive mathematical model simulating the control of the sympathetic branch of the cardiovascular system in individuals with normal and abnormal responses to the head-up-tilt test. We recruited four young women: one control, one with vasovagal syncope, one with orthostatic hypertension, and one with orthostatic hypotension, exposing them to an orthostatic head-up tilt test (HUTT) employing non-invasive methods to measure electrocardiography and continuous blood pressure.Our work encompasses a compartmental model formulated using a system of ordinary differential equations. Using heart rate as input, we predict blood pressure, flow, and volume in compartments representing the veins, arteries, heart, and the sympathetic branch of the baroreflex control system. The latter is modulated by high- and low-pressure baroreceptor afferents activated by changes in blood pressure induced by the HUTT. Sensitivity analysis, parameter subset selection, and optimization are employed to estimate patient-specific parameters associated with autonomic performance. The model has seven sensitive and identifiable parameters with significant physiological relevance that can serve as biomarkers for patient classification.Results show that the model can reproduce a spectrum of blood pressure responses successfully, fitting the trajectory displayed by the experimental data. The controller exhibits behavior that emulates the operation of the sympathetic system. These encouraging findings underscore the potential of computational methods in evaluating pathologies associated with autonomic nervous system control, warranting further exploration and novel approaches.

Quantitative Analysis of the Influence of Body Position on the Radial Pulse Wave

Abstract:Objective To reveal and quantitatively analyze the influence of postural changes on male and female radial pulse wave. Methods 20 healthy students were enrolled in this study to collect their radial pulse wave by using dual pulse signal acquisition equipment at the tilting bed with 0 degrees, 45 degrees, 90 degrees; and then, the basic hemodynamic parameters and the model parameters on dual Windkessel model were calculated. The hemodynamic parameters of male and female were analyzed and compared by paired t-test. Results The results showed that with the increase of body position (The subject along with tilt bed-from 0 degrees to 45 degrees and then to 90 degrees)both male and female heart rate (HR) and sub-endocardial myocardial viability rate (SEVR) were significant increased (P<0.05); radial artery pressure full time (Tdown) was significant decreased (P>0.05), however, other hemodynamic parameters did not change significantly with body position (P>0.05); both arterial compliance (C1) and abdominal aortic compliance (C2) became smaller with the increase of body position in male and female, peripheral resistance (R) became larger. HR, R, u-Lratio of male and female have statistical difference (P<0.05);there were no statistically significant differences in K value, Tup, Tdown, TLVET, LM-Pratio, dAI, SEVR, u-dratio, C1 and C2 between males and females (P>0.05). Conclusion Body position has an obvious influence on radial artery and there are differences between male and female hemodynamic parameters, which should be paid more attention in clinic.
Noninvasive Assessment of Temporal Dynamics in Sympathetic and Parasympathetic Baroreflex Responses

Abstract:Background: The baroreflex system is crucial for cardiovascular regulation and autonomic homeostasis. A comprehensive assessment requires understanding the simultaneous temporal dynamics of its multiple functional branches, which traditional methods often overlook. Objective: To develop and validate a noninvasive method for simultaneously assessing the temporal dynamics of sympathetic and parasympathetic baroreflexes using pulse contour analysis and the sequence method. Methods: Beat-to-beat blood pressure and ECG recordings were analyzed from 55 preoperative cardiothoracic surgery patients in the supine position and 21 subjects from the EUROBAVAR dataset in both supine and standing positions. Systolic arterial pressure (SAP), interbeat interval (IBI), cardiac output (CO), myocardial contraction (dP/dtmax), and systemic vascular resistance (SVR) were estimated using pulse contour analysis. Baroreflex sensitivity (BRS) was calculated via the sequence method and correlated with hemodynamic and heart rate variability (HRV) parameters. Results: Parasympathetic BRS for IBI was correlated with the root mean square of successive differences of ECG RR intervals (RMSSD-HRV) at 0-beat delay. Sympathetic BRS for SVR strongly correlated with SVR, CO, and RMSSD-HRV, particularly at 3-beat delay, and was uniquely associated with SAP at 1-beat delay. Sympathetic BRS for dP/dtmax correlated with dP/dtmax at 1-beat delay. In contrast, BRS for CO correlated with CO and SVR at 0- and 3-beat delays. Postural changes mainly affected parasympathetically-mediated BRS for IBI and, to a lesser extent, the sympathetic vascular and myocardial branches. Conclusions: This method effectively captures multiple baroreflex responses and their temporal dynamics, revealing distinct autonomic mechanisms and the impact of postural changes. Further validation is warranted.

New Cardiovascular Indices Based on a Nonlinear Spectral Analysis of Arterial Blood Pressure Waveforms

Abstract:A new method for analyzing arterial blood pressure is presented in this report. The technique is based on the scattering transform and consists in solving the spectral problem associated to a one-dimensional Schrödinger operator with a potential depending linearly upon the pressure. This potential is then expressed with the discrete spectrum which includes negative eigenvalues and corresponds to the interacting components of an N-soliton. The approach is similar to a nonlinear Fourier transform where the solitons play the role of sine and cosine components. The method provides new cardiovascular indices that seem to contain relevant physiological information. We first show how to use this approach to decompose the arterial blood pressure pulse into elementary waves and to reconstruct it or to separate its systolic and diastolic phases. Then we analyse the parameters computed from this technique in two physiological conditions, the head-up 60 degrees tilt test and the isometric handgrip test, widely used for studying short term cardiovascular control. Promising results are obtained.

Abstract:Systemic physiological dynamics, such as heart rate variability (HRV) and respiration volume per time (RVT), are known to account for significant variance in the blood oxygen level dependent (BOLD) signal of resting-state functional magnetic resonance imaging (rsfMRI). However, synchrony between these cardiorespiratory changes and the BOLD signal could be due to neuronal (i.e., autonomic activity inducing changes in heart rate and respiration) or vascular (i.e., cardiorespiratory activity facilitating hemodynamic changes and thus the BOLD signal) effects and the contributions of these effects may differ spatially, temporally, and spectrally. In this study, we characterize these brain-body dynamics using a wavelet analysis in rapidly sampled rsfMRI data with simultaneous pulse oximetry and respiratory monitoring of the Human Connectome Project. Our time-frequency analysis across resting-state networks (RSNs) revealed differences in the coherence of the BOLD signal and heartbeat interval (HBI)/RVT dynamics across frequencies, with unique profiles per network. Somatomotor (SMN), visual (VN), and salience (VAN) networks demonstrated the greatest synchrony with both systemic physiological signals when compared to other networks; however, significant coherence was observed in all RSNs regardless of direct autonomic involvement. Our phase analysis revealed distinct frequency profiles of percentage of time with significant coherence between BOLD and systemic physiological signals for different phase offsets across RSNs, suggesting that the phase offset and temporal order of signals varies by frequency. Lastly, our analysis of temporal variability of coherence provides insight on potential influence of autonomic state on brain-body communication. Overall, the novel wavelet analysis enables an efficient characterization of the dynamic relationship between cardiorespiratory activity and the BOLD signal in spatial, temporal, and spectral dimensions to inform our understanding of autonomic states and improve our interpretation of the BOLD signal.
The Characteristics of Cardiovascular Response to the Consecutive Body Posture Change

Abstract:Objective To observe the characteristics of cardiovascular response to the consecutive body posture change-the alternation of head up and down. Methods Eighteen healthy male subjects were exposed to the consecutive body posture change.Electrocardiogram (ECG),bloodpressure (BP),heart rate (HR),cardiac out (CO),stroke volume (SV) and total peripheral resistance (TPR) were recorded continuously. The parameters at supine posture before test were taken as the control value.We collected the cardiovascular parameters at 20 s and 60 s in each body posture for analyzing. Results There were 6 subjects of 18 had arrhythmia during the test,such as supraventricular premature beat,paroxysmal supraventricular tachycardia and atrioventricular block.The cardiovascular indexes of 18 subjects varied with posture change,eg. CO expressed decrease tendency while TPR tended to increase.There was a more profound fall on SBP,MBP,SV and CO under the 10th 15°/+50° orthostatic stress than the first -15°/+50° orthostatic stress. Conclusion The cardiovascular parameters show significant regular changes during consecutive body posture change. The pattern of cousecutive body posture change has an adverse impact on orthostatic tolerance.
Cross-correlation of instantaneous phase increments in pressure-flow fluctuations: Applications to cerebral autoregulation

Abstract:We investigate the relationship between the blood flow velocities (BFV) in the middle cerebral arteries and beat-to-beat blood pressure (BP) recorded from a finger in healthy and post-stroke subjects during the quasisteady state after perturbation for four different physiologic conditions: supine rest, head-up tilt, hyperventilation, and CO2 rebreathing in upright position. To evaluate whether instantaneous BP changes in the steady state are coupled with instantaneous changes in the BFV, we compare dynamical patterns in the instantaneous phases of these signals, obtained from the Hilbert transform, as a function of time. We find that in post-stroke subjects the instantaneous phase increments of BP and BFV exhibit well-pronounced patterns that remain stable in time for all four physiologic conditions, while in healthy subjects these patterns are different, less pronounced, and more variable. We propose an approach based on the cross-correlation of the instantaneous phase increments to quantify the coupling between BP and BFV signals. We find that the maximum correlation strength is different for the two groups and for the different conditions. For healthy subjects the amplitude of the cross-correlation between the instantaneous phase increments of BP and BFV is small and attenuates within 3-5 heartbeats. In contrast, for post-stroke subjects, this amplitude is significantly larger and cross-correlations persist up to 20 heartbeats. Further, we show that the instantaneous phase increments of BP and BFV are cross-correlated even within a single heartbeat cycle. We compare the results of our approach with three complementary methods: direct BP-BFV cross-correlation, transfer function analysis, and phase synchronization analysis. Our findings provide insight into the mechanism of cerebral vascular control in healthy subjects, suggesting that this control mechanism may involve rapid adjustments (within a heartbeat) of the cerebral vessels, so that BFV remains steady in response to changes in peripheral BP.

Abstract:The upright posture strengthens the coupling between heart period (HP) and systolic arterial pressure (SAP) consistently with a greater contribution of the arterial baroreflex to cardiac control, while paradoxically decreasing cardiac baroreflex sensitivity (cBRS). To investigate the physiological mechanisms that mediate the coupling between HP and SAP in response to different postures, we analyzed the cross-correlation functions between low-frequency HP and SAP fluctuations and estimated cBRS with the sequence technique in healthy male subjects during passive head-up tilt test (HUTT, n = 58), during supine wakefulness, supine slow-wave sleep (SWS), and in the seated and active standing positions (n = 8), and during progressive loss of 1 L blood (n = 8) to decrease central venous pressure in the supine position. HUTT, SWS, the seated, and the standing positions, but not blood loss, entailed significant increases in the positive correlation between HP and the previous SAP values, which is the expected result of arterial baroreflex control, compared with baseline recordings in the supine position during wakefulness. These increases were mirrored by increases in the low-frequency variability of SAP in each condition but SWS. cBRS decreased significantly during HUTT, in the seated and standing positions, and after blood loss compared with baseline during wakefulness. These decreases were mirrored by decreases in the RMSSD index, which reflects cardiac vagal modulation. These results support the view that the cBRS decrease associated with the upright posture is a byproduct of decreased cardiac vagal modulation, triggered by the arterial baroreflex in response to central hypovolemia. Conversely, the greater baroreflex contribution to cardiac control associated with upright posture may be explained, at least in part, by enhanced fluctuations of SAP, which elicit a more effective entrainment of HP fluctuations by the arterial baroreflex. These SAP fluctuations may result from enhanced fluctuations of vascular resistance specific to the upright posture, and not be driven by the accompanying central hypovolemia.
Effects of Different Stresses on Cardiac Autonomic Control and Cardiovascular Coupling

Abstract:The objective of this study was to investigate the impacts of different stresses on time-varying autonomic reactivity and cardiovascular coupling. In total, 25 male subjects were recruited. RR intervals (RRI), systolic and diastolic blood pressure (SBP, DBP), stroke volume (SV), cardiac output (CO), and systemic vascular resistance (SVR) values were collected during rest, mental arithmetic task (MAT), and cold pressor test (CPT). Baroreflex sensitivity (BRS) was derived using the transfer function method. Continuous wavelet transformation of RRI was used to describe the time-variant patterns of autonomic neural activities. Wavelet cross correlation and phase synchronization were used to estimate the amplitude and phase coupling between RRI and SBP. MAT was characterized by increased heart rate (HR), SBP, DBP, and CO with decreased BRS attributable to prolonged parasympathetic withdrawal. Moreover, cardiovascular coupling was disrupted in MAT. These results indicated that baroreflex was depressed, and the top-down system started to take action under mental stress. In CPT, SBP, DBP, and SVR increased significantly, whereas HR and BRS remained unchanged. The increase of sympathetic activity was transient, and cardiovascular coupling did not change in CPT. Intriguingly, the frequency of the maximum cross-correlation coefficient in the low-frequency band (0.04-0.15 Hz) was significantly decreased in CPT, which may be due to the change of resonance frequency of the baroreflex loop.NEW & NOTEWORTHY The study is the first to compare the time-variant pattern of autonomic nervous activities and cardiovascular coupling between the mental arithmetic task (MAT) and the cold pressor test (CPT). Our results demonstrated that MAT and CPT elicited different time-varying patterns of autonomic neural activities and cardiovascular synchronization. Both the amplitude and phase consistency of blood pressure and heart rate decreased in MAT. CPT may affect the harmonic frequency of the baroreflex loop.
The Effects of Change of Body Position on Autonomic Nervous Regulation of the Cardiovascular System Exam-Ined by Synthesis Echo Coefficient

Abstract:Objective Through studying the sensitivity,reliability and correlation among heart rate variability(HRV),blood pressure variability(BPV) and synthesis echo coefficient(SEC) forevaluation of autonomic nervous regulation of the cardiovascular system in various body positions,so as to detect whether SEC can be used as a new simple reliable noninvasive parameter for researches in space medicine.Methods Ten healthy male volunteers served as subjects.They were asked to keep in sitting position(Z) for five minutes,the first supine position(P_1) for ten minutes,head-down tilt test(HDT 15°) for ten minutes,the second supine position(P_2) for ten minutes and +75°passive head-up tilt test(HUT) for ten minutes.In the meantime,their arterial blood pressure was measured by the LXJ12 blood pressure mercury manometer,and cardiovascular function was measured with the SZ-II synchronism noninvasion examination analysis system for left-right cardiac function and Cardio-Win ECG workstation.Results The HRV increased and the arterial systolic blood pressure variability(SBPV) and SEC deceased during P_1,HDT and P_2 as compared with during Z.The HRV decreased significantly,but SBPV and SEC increased significantly during HUT.Conclusions HRV,SBPV as well as SEC can equally reflect the autonomic nervous regulation of the cardiovascular system during change of body position.As a new simple reliable noninvasive parameter,the perspective of application of SEC in space medicine researches is promising.
Characterization of autonomic states by complex sympathetic and parasympathetic dynamics

Abstract:Assessment of heartbeat dynamics provides a promising framework for non-invasive monitoring of cardiovascular and autonomic states. Nevertheless, the non-specificity of such measurements among clinical populations and healthy conditions associated with different autonomic states severely limits their applicability and exploitation in naturalistic conditions. This limitation arises especially when pathological or postural change-related sympathetic hyperactivity is compared to autonomic changes across age and experimental conditions. In this frame, we investigate the intrinsic irregularity and complexity of cardiac sympathetic and vagal activity series in different populations, which are associated with different cardiac autonomic dynamics. Sample entropy, fuzzy entropy, and distribution entropy are calculated on the recently proposed sympathetic and parasympathetic activity indices (SAI and PAI) series, which are derived from publicly available heartbeat series of congestive heart failure patients, elderly and young subjects watching a movie in the supine position, and healthy subjects undergoing slow postural changes. Results show statistically significant differences between pathological/old subjects and young subjects in the resting state and during slow tilt, with interesting trends in SAI- and PAI-related entropy values. Moreover, while CHF patients and healthy subjects in upright position show the higher cardiac sympathetic activity, elderly and young subjects in resting state showed higher vagal activity. We conclude that quantification of intrinsic cardiac complexity from sympathetic and vagal dynamics may provide new physiology insights and improve on the non-specificity of heartbeat-derived biomarkers.
Assessment of the Coupling Between Heart Rate and Arterial Pressure During Head-Up Tilt

Abstract:This study is aimed to investigate the changes of the coupling strength between RR interval (RRI) and systolic blood pressure (SBP) before and after head-up tilt (HUT) with different tilt speeds from dynamic and steady perspectives.The data used was from database Physiologic Response to Changes in Posture(PRCP) published on PhysioNet,providing documentary ECG and continuous arterial blood pressure signals of ten healthy subjects (5 males and 5 females) during HUT stimulation.Beat-by-beat time series of RRI and SBP were extracted from both slow tilt (ST,75°HUT over 50 s) and rapid tilt (RT,75°HUT over 2 s).Then,timefrequency analysis and information decomposition analysis,combined with time-domain indexes and short-term fractal exponent (α1) were applied to perform joint analysis between RRI and SBP.The results of information decomposition analysis indicated that all of the significant differences appeared in the feedback direction (SBP →RRI) due to baroreflex control on RRI.The prediction of RRI after ST significantly increased compared to that in supine position (0.416 ± 0.067 vs 0.626 ± 0.127),indicating the elevation of the coupling strength along the baroreflex.However,HUT showed few effects in the feedforward direction of RRI→SBP.There wereno significant differences between ST and RT for all of the same indexes before HUT.However,the coefficient of variation of RRI (CVRRI) in the steady state after RT was significantly increased and α1 was significantly decreased compared to that after ST despite the fact that there was no difference for RRI.What's more,the results of time-frequency analysis suggested the different behavior of dynamic response to ST and RT.Our research proved the effectiveness of information decomposition analysis to detect the dominant causal direction (feedback or feedforward) in the RRI-SBP interactions and to characterize the changes of the prediction of RRI and SBP signal before and after HUT.

Abstract:Purpose Orthostasis increases the variability of continuously recorded blood pressure (BP). Low-frequency (LF) BP oscillations (Mayer waves) in this setting are related to the vascular-sympathetic baroreflex. Mechanisms of increased high-frequency (HF) BP oscillations at the periodicity of respiration during orthostasis have received less research attention. A previously reported patient with post-neurosurgical orthostatic hypotension (OH) and vascular-sympathetic baroreflex failure had large tilt-evoked, breathing-driven BP oscillations, suggesting that such oscillations can occur independently of vascular-sympathetic baroreflex modulation. In the present study we assessed effects of orthostasis on BP variability in the frequency domain in patient cohorts with or without OH. Methods Power spectral analysis of systolic BP variability was conducted on recordings from 73 research participants, 42 with neurogenic OH [13 pure autonomic failure, 14 Parkinson's disease (PD) with OH, 12 parkinsonian multiple system atrophy, and 3 status post-brainstem neurosurgery] and 31 without OH (control group of 16 healthy volunteers and 15 patients with PD lacking OH), before, during, and after 5′ of head-up tilt at 90 degrees from horizontal. The data were log transformed for statistical testing. Results Across all subjects, head-up tilting increased HF power of systolic BP variability ( p = 0.001), without a difference between the neurogenic OH and control groups. LF power during orthostasis was higher in the control than in the OH groups ( p = 0.009). Conclusions The results of this observational cohort study confirm those based on our case report and lead us to propose that even in the setting of vascular-sympathetic baroreflex failure orthostasis increases HF power of BP variability.

Abstract:This paper aims to understand human skin blood flow(SBF) response to external pressure. Alternative pressure loading by using sphygmomanometer was applied on calf while subjects sitting and lying supine respectively; the SBF was observed with laser flowmeter and wavelet analysis was carried out on the SBF signals. The main results are as follows: under pressure of 20mmHg and 40mmHg, SBF obviously increased with sitting posture while that of some subjects turned to decrease under pressure of 40mmHg when lying supine; the frequency of myogenic activity decreased under pressure. It was explained that the SBF change under pressure is the result of balance between the increase through the capillary recruitment and the decrease due to mechanical squashing of small vessels under high pressure; the turning point between increase and decrease depends on the blood pressure inside the arterioles.
