Change in physical activity outcomes versus global rating of change scale

Abstract:Background: Intervention effect is expressed by change in valid health outcomes. These normaly relate with patients' perspective by a global rating of change (GRC) scale for the construct of the outcome. Aim: To compare the change in physical activity (PA) outcomes to a GRC scale in people with COPD following a 1-year PA coaching intervention and its relation to the MCID. Methods: Pooled data from an RCT (NCT04139200) comparing a 1-year intensive or light PA coaching intervention were used. Accelerometry based PA was measured by the Dynaport Movemonitor at baseline and 12 months follow-up. Experienced amount of and difficulty with PA was measured using the C-PPAC instrument. Perception of change in PA was captured by a 7-point GRC-scale. Results: Data was collected in 125 patients with COPD (age 67±7, 38% male, FEV1 58±16%pred, 5482±3004 steps/day). There was no difference in GRC-scale between intervention groups (p=0.77). Low Spearman correlations between the GRC-scale and PA related outcomes were observed (steps per day: r=0.31; C-PPAC difficulty: r=0.31; C-PPAC amount: r=0.29 (p<0.05 for all)) with generally an overestimation in GRC compared to PA outcomes (Fig 1). Conclusion: There is a discrepancy between the change in PA related outcomes and perceived change in PA by patients, potentially related to the long recall time. Changes between GRC levels are in line with the MCID for C-PPAC difficulty and approaches the MCID for steps per day.
