Abstract:1666 Objectives: Dementia with Lewy bodies (DLB) shares many similar aspects with Parkinson’s disease dementia (PDD) and Alzheimer’s disease (AD). Distinction between these diseases becomes important from the viewpoint of clinical treatment and pharmacological trials. Previous MRI studies indicated that the brain network organizations of these diseases are different1,2. In this study, we investigated the difference of the topological architecture of whole-brain connectivity in different dementias by using 18F-FDG PET imaging. Methods: Twenty-two patients with clinically probable DLB, eighteen probable PDD, twenty-two probable AD and twenty-two healthy controls (HC) were recruited to perform 18F-FDG PET imaging in resting state. It is assumed that brain regions whose cerebral metabolic rate for glucose (CMRgl) values are significantly correlated are functionally associated and the local CMRgl in a set of 90 structures belonging to the AAL atlas was obtained. The glucose metabolism network (represented by the matrix of the CMRgl covariations among all pairs of structures) was studied using the graph theory framework. Results: The highest concurrent fluctuations in CMRgl were basically identified between homologous cortical regions in all groups. Compared to the HC group, the small world characteristics of the three dementia groups was damaged, where the DLB group was most severely, the AD group followed, and the PDD group was most lightly. In term of global and local network properties, the clustering coefficients of three dementia groups were higher, the characteristic path lengths were longer and the local efficiency and global efficiency were at the lowest level in the DLB group. A direct comparison between with and without regression found that gender and age had significant effects on the brain networks. Conclusions: This study is an attempt at exploring the complex metabolic connectivity dysfunction in DLB and suggests the potential of using network topology as a diagnostic biomarker. References: 1.
