Partial Breast Reconstruction Using Thoracodorsal Flap Based on Anatomically Localized Descending Perforators Without Doppler Ultrasound: A Case Series

Abstract:Conservative surgery is considered the standard of care in early breast cancer management. Oncoplastic reconstruction is done for every tumorectomy resection that leaves a large defect. Pedicled adipo-cutaneous perforator flaps have received much attention as an alternative option with fewer donor-site morbidities. In this study, we offered thoracodorsal artery perforator (TDAP) flaps for breast-conservative surgery patients with an estimated breast volume loss of more than 20%. All consecutive patients with breast cancer up to American Joint Committee on Cancer (AJCC) stage II who presented to the Department of Surgical Oncology service were enrolled in the study from October 2016 to December 2019. Adequate resection was confirmed with a frozen-section examination, and the patients were then rotated to a lateral position, and thoracodorsal artery perforator (TDAP) flap was achieved. Twenty-seven patients were available in the 6th postoperative week to be reported. The mean patients' age was 45.5 years. Primary tumor stage ranged from Tis in one patient to T3 in four patients. Most cases (16 patients) had a T2 primary. The upper outer breast quadrant harbored the tumor in 22 patients. The central and lower outer breast regions were affected in two and three patients, respectively. Seventeen patients (63%) had excellent cosmetic scores, and seven (26%) had good scores. We concluded that raising thoracodorsal artery perforator (TDAP) flaps based on anatomical landmarks is safe and is associated with satisfactory esthetic outcomes.
