Abstract:Résumé Introduction L'annonce d'un cancer constitue un traumatisme pour le patient. En France, un dispositif d'annonce est mis en place, assurant un temps médical d'annonce et de proposition de traitement, un temps soignant d'accompagnement, sans y intégrer le pharmacien. Afin d'améliorer la prise en charge des patients traités par anticancéreux par voie intraveineuse, nous avons mis en place des consultations pharmaceutiques d'initiation en oncologie digestive. Les objectifs étaient de faire un état des lieux à un an de la mise en place de ces consultations et d'évaluer leur apport. Méthodes Lorsqu'un patient était diagnostiqué d'un cancer digestif pour un traitement administré par voie intraveineuse une consultation pharmaceutique d'initiation était programmée. Des indicateurs d'activité (nombre de consultations, durée moyenne, temps de préparation moyen et différents délais), de résultats (nombre et type d'intervention pharmaceutique, satisfaction des patients) ont été recueillis afin d'évaluer l'activité. Résultats Quarante-sept consultations pharmaceutiques d'initiation ont été réalisées. La durée moyenne de consultation était de 39,3 minutes. Les consultations étaient réalisées en moyenne 12,1 jours après la consultation médicale d'annonce et 9,6 jours avant la première cure de chimiothérapie. Vingt-neuf patients ont répondu au questionnaire de satisfaction. Ils étaient tous satisfaits et la majorité des patients déclarait avoir amélioré leurs connaissances du traitement anticancéreux. Discussion Cette activité innovante permet de reprendre avec le patient des éléments essentiels à sa prise en charge comme la pose du cathéter à chambre implantable, le traitement anticancéreux et la gestion des effets indésirables potentiels et apporte une amélioration de ses compétences d'autosoins. Summary Introduction The announcement of a cancer diagnosis is traumatic for the patient. In France, an announcement system has been in place, providing medical time for announcement and treatment proposal, nursing time for support, without including the pharmacist. In order to improve management of patients treated with intravenous anticancer drugs, we set up introductory pharmaceutical consultations in digestive oncology. The aims were to assess the situation one year after the introduction of these consultations, and to assess their contribution. Methods When a patient was diagnosed with digestive cancer and receiving intravenous treatment, a pharmaceutical initiation consultation was scheduled. Indicators of activity (number of consultations, average duration, average preparation time and various delays) and results (number and type of pharmaceutical interventions, patient satisfaction) were collected in order to assess activity. Results Forty-seven pharmaceutical initiation consultations were carried out. The average duration of the consultations was 39.3 minutes. Consultations were carried out on average 12.1 days after the medical consultation and 9.6 days before the first chemotherapy treatment. Twenty-nine patients responded to the satisfaction questionnaire. All were satisfied, and the majority of patients said they had improved their knowledge of cancer treatment. Discussion This activity enables us to review with patients essential aspects of their care, such as implanting an implantable chamber catheter, anti-cancer treatment and managing potential side effects and improve their self-care skills.
