Abstract:Clinical introduction An octogenarian presented to the hospital post-fall from a bed, sustaining left facial and chest bruises without consciousness loss. He had diabetes, hypertension, stroke and atrial fibrillation, and was on aspirin 100 mg and edoxaban 30 mg daily. Vital signs were stable. Examination showed left periorbital swelling and anterior thoracic bruising. He had impaired left eye movement (figure 1A). Figure 1B depicts the opened left eye, revealing a 360° conjunctival bulge encircling the cornea. The left eye exhibited a prompt and complete pupillary light reflex. CT scans confirmed the absence of intracranial haemorrhage or orbital fracture. What is the diagnosis?
