Abstract:In this issue of JAMA Internal Medicine, an article by Himmelstein et al focuses on the potential association of wealth redistribution with longevity. As wealth inequality has grown rapidly and persisted in the US, many studies have examined the consequences of the concentration of larger proportions of monetary resources among increasingly small numbers of people at the top of the wealth scale. Amid public calls for wealth redistribution as a far-reaching approach to address inequities on multiple fronts, such as health care and public health, policymakers need corresponding empirical data on the multiple potential individual-level and population-level health benefits of economic redistributive efforts.

Association Between Racial Wealth Inequities and Racial Disparities in Longevity Among US Adults and Role of Reparations Payments, 1992 to 2018

Abstract:Importance: In the US, Black individuals die younger than White individuals and have less household wealth, a legacy of slavery, ongoing discrimination, and discriminatory public policies. The role of wealth inequality in mediating racial health inequities is unclear. Objective: To assess the contribution of wealth inequities to the longevity gap that exists between Black and White individuals in the US and to model the potential effects of reparations payments on this gap. Design, setting, and participants: This cohort study analyzed the association between wealth and survival among participants in the Health and Retirement Study, a nationally representative panel study of community-dwelling noninstitutionalized US adults 50 years or older that assessed data collected from April 1992 to July 2019. Participants included 7339 non-Hispanic Black (hereinafter Black) and 26 162 non-Hispanic White (hereinafter White) respondents. Data were analyzed from January 1 to September 17, 2022. Exposures: Household wealth, the sum of all assets (including real estate, vehicles, and investments), minus the value of debts. Main outcomes and measures: The primary outcome was all-cause mortality by the end of survey follow-up in 2018. Using parametric survival models, the associations among household wealth, race, and survival were evaluated, adjusting for age, sex, number of household members, and marital status. Additional models controlled for educational level and income. The survival effects of eliminating the current mean wealth gap with reparations payments ($828 055 per household) were simulated. Results: Of the 33 501 individuals in the sample, a weighted 50.1% were women, and weighted mean (SD) age at study entry was 59.3 (11.1) years. Black participants' median life expectancy was 77.5 (95% CI, 77.0-78.2) years, 4 years shorter than the median life expectancy for White participants (81.5 [95% CI, 81.2-81.8] years). Adjusting for demographic variables, Black participants had a hazard ratio for death of 1.26 (95% CI, 1.18-1.34) compared with White participants. After adjusting for differences in wealth, survival did not differ significantly by race (hazard ratio, 1.00 [95% CI, 0.92-1.08]). In simulations, reparations to close the mean racial wealth gap were associated with reductions in the longevity gap by 65.0% to 102.5%. Conclusions and relevance: The findings of this cohort study suggest that differences in wealth are associated with the longevity gap that exists between Black and White individuals in the US. Reparations payments to eliminate the racial wealth gap might substantially narrow racial inequities in mortality.

Abstract:This study compares the associators of healthy ageing in China and the United States, using the 2005–2014 Chinese Longitudinal Healthy Longevity Survey (CLHLS) and the 2004–2014 Health and Retirement Study (HRS). Health ageing is measured using an overall healthy ageing index (HAI), defined according to five dimensions: no major chronic diseases, free of physical functional impairment, free of cognitive impairment, no depressive symptoms, and socially active engagement. Multilevel logistic regression analysis is adopted to explore the association of demographic characteristics, socioeconomic status, and healthy lifestyles with the odds ratios of healthy ageing in China and the United States. Results indicate that the proportion of individuals experiencing healthy ageing is slightly higher in US than Chinese older adults aged 65–100 (25.67% vs 23.27%). Nevertheless, Chinese oldest-old (80–100) have a higher proportion of healthy ageing than American oldest-old (15.05% vs. 12.19%). Our results indicate that the odds of healthy ageing decrease with age, whereas they increase with education, income, marriage, and health behaviours. The odds of healthy ageing are lower for older Chinese women than men, whereas the odds are higher for older US women. Moreover, non-white elders in the United States have significantly lower odds of healthy ageing than their white counterparts. These findings suggest certain similar patterns of healthy ageing across two countries, but distinct patterns do exist. Promoting gender and racial equalities in healthy ageing are crucial for Chinese and American policymakers, respectively.
Is Redistribution Good for Our Health? Examining the Macro-Correlation between Welfare Generosity and Health across EU Nations over the Last 40 Years

Abstract:Abstract Context: Social determinants of health are finally getting much needed policy attention. Yet, their political origins remain underexplored. In this paper, we advance a theory of political determinants as accruing along three pathways of welfare state effects (redistribution, poverty reduction and status preservation) and test these assumptions by examining impacts of policy generosity on life expectancy (LE) over the last 40 years. Methods: We merge new and existing welfare policy generosity data from the Comparative Welfare Entitlement Project (CWEP) with data on LE spanning 1980-2018 across 21 OECD countries. We then examine relationships between five welfare policy generosity measures and LE using cross-sectional differencing and auto-regressive lag models. Findings: We find consistent and positive effects for total generosity (an existing measure of social insurance generosity) on LE at birth across different model specifications in the magnitude of a 0.10-0.15-year increase in LE at birth (p < 0.05) and a measure of status preservation (0.11, p < 0.05). We find less consistent support for our redistribution and poverty reduction measures. Conclusions: We conclude that in addition to generalized effects of policy generosity on health, status-preserving social insurance may be an important, and relatively overlooked, mechanism in increasing life expectancies over time in advanced democracies.

Health State Transitions and Longevity Effects on Retirees’ Optimal Annuitization

Abstract:The interplay between longevity risk and health state transitions for retirees’ optimal annuitization decisions is investigated. Using a life‐cycle framework incorporating wealth levels, bequest motives, and consumption floors created by government subsidies, we examine how increased longevity in conjunction with an individual's health state transition process impacts annuity purchase decisions. Health state transition matrices are estimated from the Health and Retirement Survey (HRS) data. The effects of increased longevity on annuitization decisions are considered when longevity is both accompanied by increased time spent in healthier states (morbidity compression) or experienced by more time in unhealthy states (morbidity expansion). We find that retirees’ annuity demand is affected by wealth, initial health status, and expansion or compression of morbidity. Wealthier retirees have higher annuity demand when health shocks are considered, and increased longevity increases demand even more when retirees expect an expansion or slight morbidity compression. With health shocks and expectations of severe morbidity compression considered, the opposite effect might occur. Thus, an annuity can help retirees hedge health shock costs when slight compression or expansion of morbidity occurs. For retirees with lower wealth, the consumption floor provided by governmental subsidies will create a decreased propensity to annuitize.

Abstract:This paper embeds a structural model of private wealth accumulation over the life cycle within a dynamic microsimulation model designed for long‐run projections of pensions. In such an environment, the optimal savings path results from consumption smoothing and bequests motives, on top of the mortality risk. Preferences are estimated based on a longitudinal wealth survey through a method of simulated moments. Simulations issued from these estimations replicate quite well a private wealth that is more concentrated than labor income. They enable us to compute "augmented" standards of living including capital income, hence to quantify both the countervailing role played by private wealth to earnings dropout after retirement and the impact of the mortality risk in this regard.

Abstract:We document in US data that returns to wealth across households are significantly heterogeneous, and persistently so. Motivated by this observation, we build a tractable general equilibrium model where households face persistent idiosyncratic returns to study the US wealth distribution. We show theoretically that the wealth distribution in the model admits a Pareto tail and characterize how the tail index depends on salient equilibrium variables including capital-output ratio, labor share, interest rate, and growth rate. Quantitatively, to match the observed US wealth distribution it requires significant heterogeneity in returns, consistent with our empirical findings. Finally, we show in the model that financial deregulation and a reduction in US corporate tax rates can generate the joint evolution of rising wealth inequality, rising capital-output ratio and declining labor share since the 1980s.

Abstract:Journal of Aging and Health, Ahead of Print. ObjectivesTo assess how cognitive trajectories from mid-to-later life relate to wealth change, overall and by mid-life income. Methods: Data were from participants (51–64 years) in the 2000–2018 U.S. Health and Retirement Study who were cognitively healthy at baseline (year 2000; unweighted n = 3821). Longitudinal latent class analyses generated cognitive and wealth trajectories, independently, and multinomial logistic regressions estimated the association between cognitive trajectories and wealth profiles, overall and by median income. Results: We identified three cognitive: cognitively healthy (CH), increasing cognitive impairment (ICI), and increasing dementia (ID) and four wealth profiles: stable wealth loss (SWL), delayed gradual wealth loss (DGWL), stable wealth gain (SWG), and gradual wealth gain (GWG). The ID group had higher probability of being in the SWL group and lower probability of SWG, which was more pronounced in respondents with greater median income. Discussion: Individuals with ID may be vulnerable to wealth loss, particularly for middle-class households.

Abstract:Many low-income households in rich countries have very little wealth, but the role of intergenerational wealth transmission in underpinning this deficit is not known. This article seeks to fill that gap by investigating patterns of past wealth transfer receipt for low-income versus other households in seven rich countries and assessing the contribution that these transfers, or their absence, make to current wealth levels. We find that households on low incomes are relatively disadvantaged in terms of intergenerational transfers received in the past, both in terms of the likelihood of having received any and the amounts received by those who do benefit from such transfers. The role that this disadvantage plays in the linkage between current low-income and low wealth is assessed and evidence presented that it is significant. Simulation of a universal wealth transfer scheme or ‘capital endowment’ on reaching adulthood for two countries shows that such a policy could lead to a marked decline in the proportion of low-income adults with negative or no wealth. This and alternative or complementary policy responses to these wealth deficits merit the most serious attention.

Abstract:A large body of literature has investigated the health-wealth nexus. Most studies exploit exogenous shocks to wealth, such as lottery prizes and inheritances, to investigate the causal effects of wealth on health, especially in developed countries. This paper draws on data from the China Health and Retirement Longitudinal Study (CHARLS) to first examine the impacts of wealth on the health of the elderly in China. Following the method of Lovenheim and Mumford (2013), I use growth in household housing wealth driven by prefecture-level housing market as an exogenous variation in household-level wealth in China. The findings indicate that wealth has causal effects on individual health-related outcomes. Surprisingly, the growth in housing wealth imposes mixed effects on health conditions and, in fact, reduces healthy activities and healthcare utilisation. As their housing wealth increases, people with lower education levels generally suffer worse physical health and engage in fewer healthy behaviours than those with higher education levels, making health literacy a possible explanation for the mixed health-wealth nexus for the elderly in China.

Abstract:We show that genetic endowments linked to educational attainment strongly and robustly predict wealth at retirement. The estimated relationship is not fully explained by flexibly controlling for education and labor income. We therefore investigate a host of additional mechanisms that could account for the gene-wealth gradient, including inheritances, mortality, risk preferences, portfolio decisions, beliefs about the probabilities of macroeconomic events, and planning horizons. We provide evidence that genetic endowments related to human capital accumulation are associated with wealth not only through educational attainment and labor income, but also through a facility with complex financial decision-making.

Abstract:We present, solve and numerically simulate a simple model that describes the consequences of increased longevity on fertility rates, population growth and the distribution of wealth in developed societies. We look at the consequences of the repeated use of life extension techniques and show that they represent a novel commodity whose introduction will profoundly influence key aspects of economy and society in general. In particular, we uncover two phases within our simplified model, labeled as 'mortal' and 'immortal'. Within the life extension scenario it is possible to have sustainable economic growth in a population of stable size, as a result of dynamical equilibrium between the two phases.

Abstract:This paper investigates the wealth, consumption, and savings behavior of elderly individuals through the lens of the life-cycle hypothesis. Using data from Survey of Health, Ageing and Retirement, we employ Student's t-test to test differences between pre-retirement and retirement age groups. The findings shed light on the extent to which elderly individuals adhere to the life-cycle hypothesis and provide insights into the economic behavior of this demographic group. Our findings align with previous studies indicating that the life-cycle theory inadequately captures the dynamics of wealth, consumption, and savings during retirement age. According to our results, there is a consistent decrease in all examined variables with advancing age, with the lowest yet non-zero values observed among the oldest individuals.

Key Message This study found wealth-related disparities in the end-of-life experiences of older adults, with lower wealth linked to a higher likelihood of experiencing 6+ end-of-life symptoms. Medicaid enrollment seems to mitigate this disparity, suggesting certain programs can reduce these differences. Importance Prior research examined health disparities in end-of-life (EOL) based on race, ethnicity and college education. This study examines wealth-related disparities in EOL experience among older adults. Objective(s) Our study aims to: (1) Assess EOL symptom prevalence and associations with wealth; (2) Determine if wealth's effect on EOL symptoms is modified by hospice or Medicaid enrollment. Scientific Methods Utilized The study included 9,509 Health and Retirement Study (HRS) participants who died between 2000 and 2020 at age 65+. Wealth was measured by RAND household net worth variable at the last core interview before death4,5, categorized into bottom quartile, two middle quartiles, and top quartile. End-of-life symptoms, reported by proxies at the exit interview, included 10 symptoms. Our primary outcome was a binary variable, indicating presence of 6+ EOL symptoms. Results The mean age at death was 81 (SD 9); 55% women, 83% White, and 15% college graduates. Overall, 28% of descendants experienced 6+ EOL symptoms. Those with low wealth had higher likelihood of 6+ EOL symptoms compared to middle or high wealth groups (36% vs. 28% vs. 23%, p< 0.001). These differences persisted after adjusting for age, gender, marital status, race or ethnicity, and college education (low-middle wealth: aOR=0.69 (CI: 0.60-0.78); low-high wealth: aOR=0.56 (CI: 0.47-0.65)). There was no significant difference in the effect of wealth among those enrolled in hospice. Medicaid enrollment seemed to diminish the difference between low and middle wealth groups (not in Medicaid: aOR=0.74 (CI: 0.59-0.95); in Medicaid: aOR=0.90 (CI: 0.71-1.12), p for interaction=0.133). Conclusion(s) Lower wealth at the end of life is associated with worse EOL experience. However, enrollment in Medicaid decreases the difference between low and middle wealth groups. Impact Our findings suggest that Medicaid may alleviate wealth-related end-of-life differences. Further studies are needed to determine if the wealth directly impacts EOL outcomes, or if its effects are mediated by functional decline. Abstract:<h2>Abstract</h2><p>In this paper, we study alternative retirement reforms designed to achieve fiscal sustainability in the face of demographic change. We are particularly interested in the heterogeneous effects across demographic groups, as improvements in health and longevity have not been uniform across the population. To this end, we develop a dynamic, structural life cycle model of heterogeneous agents who face health, mortality and income risk. We consider the following policy reform measures: (1) increasing the early access age to old-age retirement, (2) raising income taxes, (3) lowering old-age retirement benefits and (4) lowering old-age retirement and disability benefits. We find that, of the considered policies, proportionally lowering old-age retirement and disability benefits results in the highest average welfare for all education categories. It is also the most successful at boosting employment.</p>

Endogenous longevity and optimal tax progressivity

Abstract:We study the impact of endogenous longevity on optimal tax progressivity and inequality in an overlapping generations model with skill heterogeneity. Higher tax progressivity decreases both the longevity gap and net income inequality, but at the expense of lower average lifetime and income. We find that the welfare-maximizing income tax is less progressive in our model with endogenous longevity than in our model with exogenous longevity. In a highly stylized calibration of the US economy, we show that optimal tax progressivity is less than what prevails under the current US tax system. Our results are robust to the range of empirical labor supply elasticity and the assumptions of missing annuity markets and stochastic health. Our conclusion for the optimal progressivity of the US tax system can be altered by the adoption of a more egalitarian welfare function or by increases in prevailing levels of wage inequality.

Abstract:Rising income and wealth inequality across the developed world has prompted a renewed focus on the mechanisms driving inequality. This paper contributes to the existing literature by studying the impact from life-cycle savings, intergenerational transfers, and fertility differences between the rich and the poor on the wealth distribution. We find that bequests increase the level of wealth inequality and that fertility differences between the rich and the poor amplify this relationship. The counterfactual exercises show that the interaction between bequests and differential fertility is quantitatively important for understanding wealth inequality in the United States.

Abstract:This paper investigates the ability of subjective expectations about life expectancy to predict wealth holding patterns in later life. Based on panel data from the Asset and Health Dynamics among the Oldest Old, we estimate a structural life-cycle model with bequests. Each individual's subjective survival rates in the future are estimated with data on his belief of survival probabilities to a target age. This estimation is build upon a Bayesian updating method developed in Gan et al. (2005). We find that life-cycle model using subjective survival rates performs better than using life-table survival rates in predicting wealth holdings. This result suggests that subjective survival expectations play an important role in deciding consumption and savings. In addition, the estimation results show that most bequests are involuntary or accidental.
Association of Social Mobility With the Income-Related Longevity Gap in the United States: A Cross-Sectional, County-Level Study

Abstract:Importance: Despite substantial research, the drivers of the widening gap in life expectancy between rich and poor individuals in the United States-known as the longevity gap-remain unknown. The hypothesis of this study is that social mobility may play an important role in explaining the longevity gap. Objective: To assess whether social mobility is associated with income-related differences in life expectancy in the United States. Design, setting, and participants: This cross-sectional, ecological study used data from 1559 counties in the United States to assess the association of social mobility with average life expectancy at age 40 years by sex and income quartile among adult men and women over the period of January 2000 through December 2014. Bayesian generalized linear multilevel regression models were used to estimate the association, with adjustment for a range of socioeconomic, demographic, and health care system characteristics. Exposures: County-level social mobility, here operationalized as the association of the income rank of individuals born during the period of January 1980 through December 1982 (based on tax record data, averaged over the period January 2010 through December 2012) with the income ranks of their parents (averaged over the period January 1996 through December 2000) using the location where the parent first claimed the child as a dependent at age 15 years to identify counties. Main outcomes and measures: The main outcome was life expectancy at age 40 years by sex and income quartile. Results: The sample consisted of 1559 counties, which represented 93% of the US population in 2000. Each 1-SD increase in social mobility-equivalent to the difference between a low-mobility state, such as Alabama (ranked 49th on this measure), and a higher-mobility state, such as Massachusetts (ranked 23rd on this measure)-was associated with a 0.38-year (95% credible interval [CrI], 0.29-0.47) and a 0.29-year (95% CrI, 0.21-0.38) increase in county-level life expectancy among men and women, respectively, in the lowest income quartile. Estimates for life expectancies among county residents in the highest income quartile were smaller in magnitude and not robust to covariate adjustment (men: 0.10-year [95% CrI, -0.02 to 0.22] increase; women: 0.08-year [95% CrI, -0.05 to 0.20] increase). Increasing social mobility in all counties to the value of the highest social mobility county was associated with decreases in the life expectancy gap between the highest and lowest income quartiles by 1.4 (95% CrI, 0.7-2.1) years for men and 1.1 (95% CrI, 0.5-1.6) years for women nationally, representing a 20% decrease. Conclusions and relevance: In this cross-sectional study, higher county-level social mobility was associated with smaller county-level gaps in life expectancy by income. These findings motivate further investigation of causal relationships between policies that shift social mobility and health outcomes.
