Abstract:Acute-on-chronic liver failure (ACLF) is an acute syndrome accompanied with decompensation of cirrhosis, organ failure with high 28-day mortality rate. Systemic inflammation is the main feature of ACLF, and poor outcome is closely related with exacerbated systemic inflammatory responses. It is well known that severe systemic inflammation is an important event in chronic hepatitis B (CHB)-ACLF, which eventually leads to liver injury. However, the initial CHB-ACLF events are unclear; moreover, the effect of these events on host immunity as well as that of immune imbalance on CHB-ACLF progression are unknown. Here, we investigate the initial events of ACLF progression, discuss possible mechanisms underlying ACLF progression, and provide a new model for ACLF prediction and treatment. We review the characteristics of ACLF, and consider its plausible immune predictors and alternative treatment strategies.

What problem does this paper attempt to address?
The paper attempts to address the initial events of acute exacerbation of chronic hepatitis B (CHB) to acute-on-chronic liver failure (ACLF) and its impact on host immunity, as well as the mechanisms by which immune imbalance contributes to the progression of CHB-ACLF. Specifically, the authors explore the following aspects: 1. **Initial Events**: The paper investigates the initial events of CHB-ACLF, particularly how these events affect the host's immune system and lead to immune imbalance. 2. **Immune Mechanisms**: The paper provides a detailed analysis of the immune system dysfunction in CHB-ACLF, including innate immune recognition, cytokine storm, and immune cell dysfunction. 3. **Potential Triggers**: The paper examines potential triggers that may lead to CHB-ACLF, such as HBV genotype, viral mutations, and host immune pressure. 4. **Prediction and Treatment Strategies**: The paper proposes early indicators for predicting CHB-ACLF and possible treatment strategies to reduce the incidence and mortality of the disease. Through these studies, the authors hope to provide new insights and methods for the early diagnosis and treatment of CHB-ACLF.
