Abstract:Alcohol use disorder (AUD) is a significant global health concern, leading to approximately 3 million preventable deaths annually. 1 AUD exists on a spectrum of severity, ranging from mild to severe. Although the most commonly cited medical morbidities are those of the central nervous system 2 and liver, 3 alcohol has also been found to affect olfactory function. Several studies, including ours, have identified the presence of an olfactory deficit, both by self-reporting (subjective) and by objective testing, in individuals drinking alcohol at chronic levels compared with nondrinkers. 4-6 Further, individuals with AUD are known to have poor olfactory identification, impaired odor familiarity, and edibility judgments compared with those without AUD, 7,8 and these olfactory impairments were evident as abnormal electrophysiological recordings in individuals with AUD. 9 A working hypothesis is that these olfactory impairments could be associated with the direct actions of alcohol on central olfactory processing. Heavy and chronic alcohol consumption has been associated with structural and functional changes in the olfactory system, as well as neurodegeneration and damage to olfactory neurons, resulting in persistent olfactory deficits. 10 Such deficits may be caused by the ability of alcohol to inhibit neurogenesis, which otherwise is observed throughout life in the hippocampus and olfactory bulb. 10 Nevertheless, olfactory deficits have significant consequences, such as altered ingestive behaviors, dangers from the inability to detect gas leaks or to detect spoiled food, and a lower quality of life. 4 Detailed understanding of olfactory function in AUD populations can help alleviate challenges in their daily activities, thereby enhancing their overall quality of life.
